基于“肾络瘀毒痹阻论”探析降肌汤治疗糖尿病肾病的疗效机制

摘　　要：糖尿病肾病(diabetic nephropathy,DN)归属于中医“消渴肾病”范畴,是消渴病日久不愈所并发的疑难病症。中医各家对于DN的认识都离不开“瘀”,该文通过对“肾络瘀痹”理论分析以及对DN的发病机制探讨,高度总结“瘀血-瘀热-瘀毒”这一进程与DN关系密切,导师杨传经创新提出“肾络瘀毒痹阻论”,并基于此创方“降肌汤”专门针对DN这一世界难题,根据消渴肾病“本虚标实”的病性,祛邪不忘扶正,攻补兼施,辨证施治,临床疗效显著,对血清肌酐、尿素氮及蛋白尿等的治疗效果较好,以改善肾功能、减轻残余肾脏病变。文章旨在促进中医理论在临床上的运用,为中医药治疗DN提供理论依据和参考。Diabetic nephropathy(DN)belongs to the category of“Xiao Ke Nephropathy”in traditional Chinese medicine.It is a difficult disease complicated by the long-term failure of Xiao Ke disease.The understanding of DN by various Chinese medicine schools is inseparable from“stasis”.This article analyzes the theory of“kidney collateral stasis and numbness”and explores the pathogenesis of DN,and highly summarizes that the process of“blood stasis-stasis and heat-stasis and poison”is closely related to DN.The tutor Yang Chuanjing innovatively proposed the“kidney collateral stasis and numbness theory”,and based on this,created the prescription“Jiangji Decoction”specifically for DN,a world problem.According to the disease nature of Xiao Ke Nephropathy“root deficiency and superficial excess”,the evil is eliminated without forgetting to strengthen the body,attack and supplement at the same time,treat according to syndrome differentiation,and the clinical efficacy is significant.The treatment effect on serum creatinine,urea nitrogen and proteinuria is good,so as to improve renal function and reduce residual renal lesions.The article aims to promote the application of Chinese medicine theory in clinical practice and provide a theoretical basis and reference for the treatment of DN with Chinese medicine.
