基于“木郁达之,土郁夺之”探析肥胖病辨治思路

摘　　要：随着社会经济的飞速发展和相关产业的不断革新,全球居民的饮食结构和出行方式也随之发生巨大改变,与此同时,肥胖病的发病率也在全球范围内升高。肥胖病的发病机制复杂,涉及饮食、运动、行为、遗传等多方面因素,且减重后病情易反复,导致临床治疗效果并不理想。大量临床和实验研究均表明中医药治疗肥胖疗效确切,且不良反应小,不易反弹。文章以《黄帝内经》“木郁达之,土郁夺之”为理论基础,结合历代医家思想和大量临床实践,认为肥胖病的发病与先天禀赋、饮食调摄等多因素相关,主要病机为肝脾气机郁滞,升降失常,中焦郁满,以致痰湿瘀阻,病理产物郁积于经络肌肤。治疗贵在运脾升清调源流,疏肝化浊除郁积。With the rapid development of society and economy and the continuous innovation of related industries,the dietary structure and travel mode of global residents have also changed dramatically,at the same time,the incidence of obesity has also increased globally.The pathogenesis of obesity is complex,involving diet,exercise,behavior,genetics and other factors,and after weight loss,the condition is easy to repeat,resulting in clinical treatment effect is not reasonable.A large number of clinical and experimental studies have shown that the efficacy of traditional Chinese medicine(TCM)in treating obesity is accurate,with small side effects and not easy to rebound.This article is based on The Inner Canon of the Yellow Emperor(《黄帝内经》)“venting the wood constraint,removing the earth stagnation”as the theoretical basis,combined with the ideas of medical doctors and a large number of clinical practice,think that The onset of obesity is related to multiple factors such as innate endowment and dietary regulation,and the main pathogenesis is stagnation of liver and spleen,irregular lifting and lowering,and the Middle-Jiao is full,resulting in phlegm and phlegm,and the products of the pathology are deposited in the meridians and the skin.The treatment is to transport the spleen to elevate clear and regulate the source,dredge the liver to eliminate turbidity and get rid of the accumulation.
