摘　　要：《临证指南医案》一书共10卷,其中卷9为调经篇,对女子月经疾病的调治颇具特色。叶天士提出“女子以肝为先天”理论,认为女子的生理病理与肝关系密切。归纳叶氏以肝为核心论治月经病的辨治思路为以下方面:疏肝解郁,宣疏气血;调肝理脾,土木相安;泄肝通胃,以通为补;疏肝泄火,顺气导血;柔肝化瘀,辛润通络;温补肝肾,暖益冲任;填补肝肾,滋阴固冲。基于以上辨治思想,探究叶氏临证从肝论治月经病的理论内涵和治疗用药之根本,为现代临床治疗月经病提供参考。The book“Clinical Guide Medical Cases”has a total of 10 volumes,among which Volume 9 is about regulating menstruation,which is quite distinctive in regulating and treating women’s menstrual diseases.YE Tianshi proposed the theory that“women are born with liver”,and believed that women’s physiological pathology is closely related to the liver.The following aspects can be summarized in YE’s treatment ideas of menstrual diseases with liver as the core:soothing the liver and relieving depression,dispersing Qi and blood;regulating the liver and spleen,and making the earth and wood harmonious;relieving the liver and stomach,and using unblocking as a tonic;soothing the liver and relieving fire,smoothing qi and guiding blood;softening the liver and removing blood stasis,and moistening and unblocking the collaterals;warming and nourishing the liver and kidney,warming and benefiting Chong and Ren;replenishing the liver and kidney,and nourishing Yin and consolidating Chong.Based on the above treatment ideas,the theoretical connotation and fundamental treatment of YE’s treatment of menstrual diseases from the liver are explored,providing a reference for the modern clinical treatment of menstrual diseases.
