吴昆诊治“痹症”思路及特色探析

摘　　要：痹症是一种以肌肉关节疼痛或活动受限为主的疾病,发病多因风寒湿邪侵犯人体,囊括西医骨性关节炎、类风湿性关节炎、强直性脊柱炎等众多疾病,发病人群众多,严重影响患者生活质量。明代著名医家吴昆以注释《内经》闻名,晚年结合《内经》理论及自身行医经历编写完成《针法六集》,其中《尊经集》以《内经》为理论基础,记录吴昆对痹症的证治分类及治疗方法,吴昆认为,痹症成因与风寒湿邪侵犯人体肌肉筋骨有关,治法应随病位不同而改变。针对寒痹,吴昆着重强调了在外“燔针劫刺”、在内“熨引饮药”的不同治法,其特色的用针方法、对痹症多角度解析对现今临床诊治具有一定指导意义,值得广大医者进一步研究。Bi syndrome is a disease characterized by muscle and joint pain or limited mobility.It is mostly caused by wind,cold and dampness invading the human body.It includes many diseases such as osteoarthritis,rheumatoid arthritis,ankylosing spondylitis,etc.in Western medicine.There are many people who suffer from it,which seriously affects the quality of life of patients.WU Kun,a famous doctor in the Ming Dynasty,was famous for his annotations to the“Inner Canon of Medicine”.In his later years,he combined the theory of the“Inner Canon of Medicine”and his own medical experience to complete the“Six Collections of Acupuncture Methods”.Among them,the“Zun Jing Ji”is based on the“Inner Canon of Medicine”and records WU Kun’s classification and treatment methods of Bi syndrome.WU Kun believes that the cause of Bi syndrome is related to the invasion of wind,cold and dampness into the muscles,tendons and bones of the human body,and the treatment method should change with the different locations of the disease.For cold Bi syndrome,WU Kun emphasized the different treatment methods of“burning needles and pricking”on the outside and“ironizing and drinking medicine”on the inside.His unique needle method and multi-angle analysis of Bi syndrome have certain guiding significance for today’s clinical diagnosis and treatment,and are worthy of further study by doctors.
