浅谈陆芝兰分期论治亚急性甲状腺炎临证心得

摘　　要：亚急性甲状腺炎是一种常见的甲状腺疼痛性质疾病,属于中医学“瘿病”范畴。西医主要采用非甾体抗炎药、糖皮质激素等治疗,但不良反应较多,经济花费较大,且调护不当容易转变为永久性甲状腺功能减退症,影响甲状腺功能的恢复,中医学在治疗此病上有着显著优势。陆芝兰教授根据自己多年临床经验,将该病分为热毒伤络期、气郁痰阻期、气阴亏虚期,分别灵活采用疏风清热、理气化痰、益气养阴等治法,同时配合中药外敷,能够明显改善患者症状。举例病案1例,借此说明陆教授分期诊治亚急性甲状腺炎的独特思路及方法。Subacute thyroiditis is a common thyroid pain disease,which belongs to the category of“goiter”in traditional Chinese medicine.Western medicine mainly uses non-steroidal anti-inflammatory drugs,glucocorticoids,etc.for treatment,but there are many adverse reactions,high economic costs,and improper care can easily turn into permanent hypothyroidism,affecting the recovery of thyroid function.Traditional Chinese medicine has a significant advantage in this disease in recent years.Based on her many years of clinical experience,Professor LU Zhilan divided this disease into the heat-toxic injury stage,the Qi stagnation and phlegm obstruction stage,and the Qi-Yin deficiency stage.She flexibly used wind-clearing and heat-clearing,Qi-regulating and phlegm-resolving,Qi-invigorating and yin-nourishing treatment methods,and combined with external application of Chinese medicine,which can significantly improve the patient’s symptoms.This article takes a case as an example to illustrate Professor LU Zhilan’s unique ideas and methods for staged diagnosis and treatment of subacute thyroiditis.
