芪藤消浊颗粒对脾肾亏虚湿瘀证IgA肾病的临床干预作用

摘　　要：目的:观察芪藤消浊颗粒对脾肾亏虚湿瘀证IgA肾病患者临床疗效及血清MMP-9/TIMP-1和IL-1/IL-6的干预作用。方法:选择符合标准的66例患者,随机分为各33例的观察组和对照组,疗程中各脱落3例,最终各完成30例。2组均予缬沙坦片口服治疗,观察组加服芪藤消浊颗粒,疗程12周。观察2组患者临床疗效,检测治疗前后URBC计数、UACR、血清胱抑素C(Cys-C)、IL-1、IL-6、MMP-9和TIMP-1水平,并根据血肌酐值计算肾小球滤过率(eGFR)。结果:观察组临床疗效优于对照组(P<0.05)。治疗后,2组URBC计数、UACR、Cys-C、血清IL-1、IL-6、TIMP-1下降(P<0.05),MMP-9升高(P<0.05),观察组改善程度高于同期对照组(P<0.05)。结论:芪藤消浊颗粒对脾肾亏虚湿瘀证IgA肾病患者疗效显著,抑制炎症和抗肾纤维化可能是其作用机制。Objective:To observe the clinical efficacy of Qiteng Xiaozhuo Granules in patients with IgA nephropathy with spleen and kidney deficiency,dampness and stasis syndrome,and the intervention effect of serum MMP-9/TIMP-1 and IL-1/IL-6.Methods:A total of 66 patients who met the criteria were selected and randomly divided into an observation group and a control group with 33 cases in each group.Three cases dropped out during the treatment,and finally 30 cases were completed in each group.Both groups were treated with oral valsartan tablets,and the observation group was given Qiteng Xiaozhuo Granules for 12 weeks.The clinical efficacy of the two groups was observed,and the URBC count,UACR,serum cystatin C(Cys-C),IL-1,IL-6,MMP-9 and TIMP-1 levels were detected before and after treatment,and the glomerular filtration rate(eGFR)was calculated according to the serum creatinine value.Results:The clinical efficacy of the observation group was better than that of the control group(P<0.05).After treatment,URBC count,UACR,Cys-C,serum IL-1,IL-6,and TIMP-1 decreased in both groups(P<0.05),and MMP-9 increased(P<0.05).The improvement in the observation group was higher than that in the control group(P<0.05).Conclusion:Qiteng Xiaozhuo Granules has a significant effect on patients with IgA nephropathy with spleen and kidney deficiency,dampness and stasis syndrome.Inhibition of inflammation and anti-renal fibrosis may be its mechanism of action.
