枳实薤白桂枝汤对痰浊痹阻型不稳定性心绞痛患者血清炎性反应及血管内皮功能的影响

摘　　要：目的探讨枳实薤白桂枝汤治疗痰浊痹阻型不稳定性心绞痛患者的效果。方法选择南通市通州区中医院2022年1月—2023年12月收治的76例痰浊痹阻型不稳定性心绞痛患者为研究对象,按随机数字表法将其分为对照组和观察组,各38例。对照组采用常规治疗,观察组在对照组基础上加用枳实薤白桂枝汤治疗,两组均治疗14 d。比较两组的临床疗效、心绞痛发作情况、血清炎性反应、血管内皮功能及不良反应发生情况。结果观察组治疗总有效率为94.74%,高于对照组的78.95%,差异有统计学意义(P<0.05)。治疗后,观察组每周发作次数少于对照组,每次发作持续时间短于对照组,组间差异有统计学意义(P<0.05);观察组超敏-C反应蛋白、肿瘤坏死因子-α、白细胞介素-6水平均低于对照组,组间差异有统计学意义(P<0.05);观察组内皮素-1水平低于对照组,一氧化氮水平高于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论对痰浊痹阻型不稳定性心绞痛患者予以枳实薤白桂枝汤治疗效果确切,可改善其心绞痛发作频次,减轻血清炎性反应,调节血管内皮功能。Objective To investigate the effect of Zhishi Xiebai Guizhi Decoction on unstable angina pectoris of phlegm-turbidity-obstruction type.Methods A total of 76 patients with unstable angina pectoris of phlegm-turbidity-obstruction type admitted to Tongzhou District Hospital of Traditional Chinese Medicine of Nantong City from January 2022 to December 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 38 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with Zhishi Xiebai Guizhi decoction on the basis of the control group,both groups were treated for 14 days.The clinical efficacy,angina pectoris attack,serum inflammatory response,vascular endothelial function and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 94.74%,which was higher than 78.95%of the control group,and the difference was statistically significant(P<0.05).After treatment,the weekly frequency of attacks in the observation group was less than that in the control group,and the duration of each attack was shorter than that in the control group,the differences between the groups were statistical significance(P<0.05);the levels of hypersensitive C-reactive protein,tumor necrosis factor-αand interleukin-6 in observation group were lower than those in control group,the differences between the groups were statistical significance(P<0.05);the endothelin-1 level in the observation group was lower than that in the control group,and the nitric oxide level was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Zhishi Xiebai Guizhi decoction is effective in treating patients with unstable angina pectoris of phlegm-turbidity-obstruction typ
