膝关节镜辅助小切口内固定术对胫骨平台骨折患者膝关节功能的影响  

Impact of Knee Arthroscopy-Assisted Mini-Incision Internal Fixation on Knee Joint Function in Patients with Tibial Plateau Fracture

摘　　要：目的探讨膝关节镜辅助小切口内固定术对胫骨平台骨折患者膝关节功能的影响。方法选取2020年1月—2023年12月南通市通州区中医院收治的50例胫骨平台骨折患者为研究对象,按随机数字表法将其分为对照组和观察组,每组25例。对照组行切开复位内固定治疗,观察组行膝关节镜辅助小切口内固定治疗。比较两组的手术指标、炎性因子水平、关节功能、并发症发生情况。结果观察组手术时间、切口长度均短于对照组,术中出血量、术后引流量均少于对照组,组间差异有统计学意义(P<0.05)。术后3 d,两组肿瘤坏死因子-α、白细胞介素-8水平均高于术前,但观察组均低于对照组,差异有统计学意义(P<0.05)。随访6个月后,观察组关节功能优良率为92.00%,高于对照组的68.00%,不良反应发生率为8.00%,低于对照组的32.00%,组间差异有统计学意义(P<0.05)。结论对胫骨平台骨折患者予以膝关节镜辅助小切口内固定治疗损伤小,术后炎性反应轻,关节功能恢复良好,且并发症发生风险低。Objective To explore the impact of knee arthroscopy-assisted mini-incision internal fixation on the knee joint function of patients with tibial plateau fractures.Methods Fifty patients with tibial plateau fractures admitted to Traditional Chinese Medicine Hospital of Tongzhou District,Nantong City from January 2020 to December 2023 were selected as the research subjects,they were divided into a control group and an observation group according to the random number table method,with 25 cases in each group.The control group underwent open reduction and internal fixation,while the observation group underwent knee arthroscopy-assisted mini-incision internal fixation.The surgical indicators,levels of inflammatory factors,joint function,and occurrence of complications were compared between the two groups.Results The operation time and incision length of the observation group were shorter than those of the control group,and the intraoperative blood loss and postoperative drainage volume were less than those of the control group,and the differences between the groups were statistically significant(P<0.05).Three days after the operation,the levels of tumor necrosis factor-αand interleukin-8 in both groups were higher than those before the operation,but those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 6-month follow-up,the excellent and good rate of joint function in the observation group was 92.00%,which was higher than 68.00%in the control group,and the incidence of adverse reactions was 8.00%,which was lower than 32.00%in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion For patients with tibial plateau fractures,knee arthroscopy-assisted mini-incision internal fixation results in minimal damage,mild postoperative inflammatory response,favorable recovery of joint function,and a low risk of complications.
