膝内外侧双切口双钢板内固定术对复杂胫骨平台骨折患者骨折愈合及功能康复的影响

摘　　要：目的分析膝内外侧双切口双钢板内固定术对复杂胫骨平台骨折患者的影响。方法选取2021年3月—2024年3月金乡县人民医院收治的96例复杂胫骨平台骨折患者为研究对象,按随机数字表法将其分为对照组和观察组,每组48例。对照组采用膝前正中直切口双钢板内固定术,观察组采用膝内外侧双切口双钢板内固定术。对比两组的围术期指标、恢复情况、膝关节功能、胫骨平台角度、并发症发生情况。结果两组术中出血量对比,差异无统计学意义(P>0.05);观察组手术时间长于对照组,住院时间、骨折愈合时间、患肢负重时间均短于对照组,组间差异有统计学意义(P<0.05);术后6个月,观察组美国特种外科医院膝关节评分系统评分高于对照组,胫骨平台内翻角、胫骨平台后倾角均小于对照组,组间差异有统计学意义(P<0.05);观察组并发症发生率为4.17%,低于对照组的16.67%,差异有统计学意义(P<0.05)。结论膝内外侧双切口双钢板内固定术虽然会延长手术时间,但能够加快复杂胫骨平台骨折患者愈合时间,促进膝关节功能的恢复,对胫骨平台角度影响较小,且并发症发生较少,有一定的应用价值,值得临床借鉴。Objective To analyze the effect of internal fixation with anterior-lateral incision and double plate in the medial and lateral knee on patients with complex tibial plateau fracture.Methods A total of 96 patients with complex tibial plateau fracture admitted to Jinxiang County People's Hospital from March 2021 to March 2024 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 48 cases in each group.The control group underwent internal fixation with straight incision and double plate in the anterior midline of the knee,and the observation group underwent internal fixation with anterior-lateral incision and double plate in the medial and lateral knee.Perioperative indicators,recovery,knee function,tibial plateau angle and occurrence of complications were compared between the two groups.Results Comparison of intraoperative blood loss between the two groups,and the difference was no statistically significant(P>0.05);the operation time of observation group was longer than that of control group,the hospitalization time,the fracture healing time and the weight-bearing time of affected limb were shorter than those of control group,and the differences between the groups were statistically significant(P<0.05);after six months of operation,score of Hospital for Special Surgery Knee Score System in observation group was higher than that in control group,tibial plateau varus angle and posterior tilt of tibial plateau were smaller than those of control group,and the differences between the groups were statistically significant(P<0.05);the incidence of complications in the observation group was 4.17%,which was lower than 16.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion Although the internal fixation with anterior-lateral incision and double plate in the medial and lateral knee can prolong the operation time,it can accelerate the healing time of patients with complex tibial plateau fractures,p
