摘  要:目的探究肺炎支原体感染患儿血清金属蛋白酶组织抑制因子3(TIMP3)、信号转导和转录激活因子3(STAT3)和叉头盒蛋白M1(FOXM1)表达水平与病情严重程度的关系以及三者对重症肺炎诊断价值。方法选取2022年6月至2024年6月共162例肺炎支原体肺炎患儿为研究组,根据病情严重情况分为轻症组119例,重症组43例。另选择同期健康体检儿童162例为对照组。采用酶联免疫吸附法对所有血清样本TIMP3、STAT3和FOXM1表达水平进行检测;Pearson分析血清TIMP3、STAT3和FOXM1水平与临床数据相关性;绘制受试者工作特征(ROC)曲线评估血清TIMP3、STAT3和FOXM1水平对重症儿童肺炎支原体肺炎的诊断价值。结果与对照组相比,研究组患儿血清TIMP3表达水平下降(P<0.05),STAT3和FOXM1表达水平上升(P<0.05);随着病情严重程度增加患儿血清TIMP3表达水平进一步下降(P<0.05),STAT3和FOXM1表达水平进一步上升(P<0.05);Pearson相关性分析结果表明血清TIMP3水平与白细胞介素-2、白细胞介素-6、白细胞介素-10、白细胞介素-1β、肿瘤坏死因子-α、γ-干扰素、C反应蛋白和降钙素原呈负相关(P<0.05),STAT3和FOXM1水平与上述变量为正相关(P<0.05);ROC曲线结果表明血清TIMP3、STAT3、FOXM1三者联合诊断重症肺炎支原体肺炎患儿的曲线下面积(AUC)值为0.972,显著大于TIMP3(Z=2.581,P=0.010)、STAT3(Z=2.838,P=0.005)和FOXM1(Z=2.404,P=0.016)单独诊断。结论肺炎支原体感染患儿血清TIMP3表达水平下降,STAT3和FOXM1表达水平升高,三者表达水平与疾病严重程度具有相关性,三者联合对重症肺炎支原体感染患儿诊断价值较高。Objective To investigate the relationship between the severity of Mycoplasma pneumoniae infection and the expression levels of serum tissue inhibitor of metalloproteinases 3(TIMP3),signal transducer and activator of transcription 3(STAT3),as well as forkhead box protein M1(FOXM1)in children,and their diagnostic value for severe pneumonia.Methods A total of 162 children with Mycoplasma pneumoniae pneumonia from June 2022 to June 2024 were selected as study group.According to the severity of the disease,they were assigned into a mild group of 119 cases and a severe group of 43 cases.Another 162 children who underwent health check-up were selected as the control group.ELISA was applied to detect the expression levels of TIMP3,STAT3 and FOXM1 in all serum samples;Pearson method was applied to analyze the correlation between clinical data and serum TIMP3,STAT3 and FOXM1 levels;receiver operating characteristic(ROC)curves were plotted to evaluate the diagnostic value of serum TIMP3,STAT3 and FOXM1 levels for severe Mycoplasma pneumoniae pneumonia in children.Results Compared with the control group,the serum TIMP3 expression level in the study group was lower(P<0.05),while the STAT3 and FOXM1 expression levels were higher(P<0.05).As the severity of the disease increased,the expression level of TIMP3 in the serum of the affected children further decreased(P<0.05),while the expression levels of STAT3 and FOXM1 further increased(P<0.05).Pearson correlation analysis results showed that serum TIMP3 level was negatively correlated with the levels of IL-2,IL-6,IL-10,IL-1β,TNF-α,IFN-γ,C-reactive protein and procalcitonin(P<0.05),while STAT3 and FOXM1 levels were positively correlated with the variables mentioned above(P<0.05).ROC curve results showed that the area under the curve(AUC)value of the combination of serum TIMP3,STAT3 and FOXM1 in the diagnosis of severe Mycoplasma pneumoniae pneumonia in children was 0.972,which was obviously higher than that of TIMP3(Z=2.581,P=0.010),STAT3(Z=2.838,P=0.005)and FOXM1(Z=2.404,P=0.0
