扶正透邪解毒化瘀方对老年大鼠多重耐药铜绿假单胞菌肺炎炎症损伤及免疫功能的动态影响

摘　　要：目的 动态观察扶正透邪解毒化瘀方对老年多重耐药铜绿假单胞菌肺炎大鼠不同时相肺组织白介素-2(interleukin-2,IL-2)、白介素-4(interleukin-4,IL-4)基因和蛋白表达水平及脾、胸腺、肺组织病理变化的影响。方法 将老年SD大鼠分为空白组、模型组、西药组、中药组、中药提前给药组、中西医结合治疗组,除空白组外使用经口气管插管法进行造模;在感染后第2小时、6小时以及第1、3、5、7天取材,透射电镜观察肺组织超微结构的病理改变,酶联免疫吸附法测定肺组织IL-2、IL-4蛋白表达水平,实时荧光定量PCR法测定肺组织IL-2 mRNA、IL-4 mRNA转录水平,光镜下观察脾组织、胸腺组织病理表现。结果 与模型组和西药组相比,中药组、中药提前给药组和中西医结合治疗组均能在感染后抑制促炎因子表达(P<0.01),促进抗炎因子表达(P<0.01),减轻脾脏、肺脏病理损伤,中药提前给药组和中西医结合治疗组优势明显。结论 扶正透邪解毒化瘀方对多重耐药铜绿假单胞菌感染所致老年大鼠肺炎有明显的预防、治疗及修复作用,机制可能与减轻脾脏病理损伤,进而改善机体免疫炎症稳态,最终减轻肺脏病理损伤有关。Objective To dynamically observe the effects of Fuzheng Toujie Jiedu Huayu Decotion on interleukin-2(IL-2)and interleukin-4(IL-4)gene and protein expression in lung tissues and the pathological changes of spleen,thymus and lung tissues in elderly MultiDrug Resistant Pseudomonas Aeruginosa pneumonia rats.Methods Elderly SD rats were divided into blank group,model group,western medicine group,traditional Chinese medicine group,traditional Chinese medicine early administration group,and combination of Chinese and western medicine treatment group.In addition to the blank group,MDRPA pneumonia models were established by using oral tracheal intubation method,samples were collected on the second hours,6th hours and on the 1st,3rd,5th and 7th days after infection.The pathological changes of the ultrastructure of lung tissue were observed by transmission electron microscopy.The expression levels of IL-2 and IL-4 proteins in lung tissues were determined by ELISA,and the transcriptional levels of IL-2 mRNA and IL-4 mRNA in lung tissues were determined by RT-PCR.The pathological manifestations of spleen tissues and thymus tissues were observed under light microscope.Results Compared with the model group and the western medicine group,the traditional Chinese medicine group,the traditional Chinese medicine early administration group,and the combined treatment group could inhibit the expression of pro-inflammatory factors(P<0.01),promote the expression of anti-inflammatory factors(P<0.01),and alleviate the pathological injury of spleen and lung after infection,especially the traditional Chinese medicine early administration group and the combined treatment group had obvious advantages.Conclusion Fuzheng Toujie Jiedu Huayu Decotion has obvious prevention,treatment and repair effects on pneumonia in elderly rats caused by MDRPA infection.The mechanism may be related to the alleviation of pathological damage to the spleen,thereby improving the homeostasis of immunity and inflammation,and ultimately alleviating the pathological dam
