严继林应用温肝法治疗功能性消化不良的临床经验

摘　　要：功能性消化不良是消化系统常见和多发的功能性胃肠病,其发病率较高,严重影响患者的社会功能和生活质量。常见证候多为脾虚气滞、肝胃不和、脾胃湿热、脾胃虚寒、寒热错杂等,严继林教授认为上述证候诊治,存在部分患者疗效不理想,或收效慢、易复发的特点,结合自己的临证所得,理论结合实践,提出肝寒气郁致病的观点,将证型分为肝寒犯胃和肝寒贼脾两个证型,应用温肝法分别进行证治取得显著疗效。Functional dyspepsia is a common and prevalent functional gastrointestinal disorder of the digestive system,which has a high incidence and seriously affects patients’social functioning and quality of life.Most of the symptoms are"spleen deficiency with Qi stagnation","liver and stomach disharmony","dampness-heat of spleen and stomach","deficiency-cold of spleen and stomach","cold and heat in complexity",etc.Professor Yan Jilin believes that some patients with the above syndromes may experience unsatisfactory or slow therapeutic effect and are prone to relapse.Based on his clinical findings and the combination of theory and practice,he proposes the viewpoint of liver cold with Qi depression as the pathogenesis of functional dyspepsia.He further categorizes the syndrome into two types:liver cold invading the stomach and liver cold robbing the spleen.The warming-liver method for syndrome differentiation and treatment has yielded significant therapeutic efficacy.
