摘  要:目的了解儿童耐碳青霉烯类肠杆菌科(CRE)感染的危险因素并构建风险模型,为早期筛查高危患儿提供参考。方法通过回顾性分析和病原菌分离鉴定方法,对某医院儿科患者CRE感染危险因素进行分析并构建感染风险模型。结果多因素分析结果显示,住院天数、新生儿、入住ICU、使用3代头孢构成儿童感染CRE的独立危险因素。对绘制的风险列线图进行验证,校准曲线与理想曲线拟合较好,所构建的模型具有较好的预测性。结论本研究所构建的风险列线图模型能较好地预测儿童CRE感染风险。Objective To investigate the risk factors for carbapenem-resistant Enterobacteriaceae(CRE)infection in children and construct a risk model,in order to provide reference for early screening of high-risk children.Methods Through retrospective analysis and pathogen isolation and identification methods,the risk factors for CRE infection in pediatric patients of a hospital were analyzed,and an infection risk model was constructed.Results Multivariate analysis showed that hospitalization days,newborns,ICU admission,and use of third-generation cephalosporins were independent risk factors for CRE infection in children.The constructed risk nomogram was verified and the results showed that the calibration curve was fitted well to the ideal curve,indicating that the constructed model had a good predictability.Conclusion The risk nomogram model constructed in this study can effectively predict the risk of CRE infection in children.
