摘 要:目的探讨纤维支气管镜肺泡灌洗对ICU肺部感染患者呼吸动力学指标及炎性因子的影响。方法选取2022年3月至2024年3月医院ICU收治的100例肺部感染患者作为研究对象,按照治疗方式的不同分为对照组和试验组,每组50例。对照组采用常规治疗(抗感染药物、雾化吸入及营养支持),在此基础上,试验组同时联合纤维支气管镜治疗。比较两组的呼吸动力学指标[气道阻力(Raw)、气道峰压(PIP)和动态顺应性(Cdyn)]、炎症因子[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)]水平、康复效果及临床疗效。结果治疗14 d后,两组Raw、PIP水平低于治疗前,Cdyn水平高于治疗前(P<0.05),试验组Raw、PIP水平低于对照组,Cdyn水平高于对照组(P<0.05);两组hs-CRP、TNF-α、PCT水平低于治疗前(P<0.05),试验组hs-CRP、TNF-α、PCT水平低于对照组,差异有统计学意义(P<0.05);试验组机械通气时间、肺部感染改善时间及ICU住院时间短于对照组(P<0.05);试验组总有效率高于对照组(P<0.05)。结论纤维支气管镜肺泡灌洗可以改善ICU肺部感染患者呼吸动力学指标,提高肺功能,抑制炎症反应,促进患者的康复。Objective To investigate the effects of fiberoptic bronchoscopy bronchoalveolar lavage on respiratory dynamics and inflammatory factors in ICU patients with pulmonary infections.Methods A total of 100 patients with pulmonary infections admitted to the ICU of a hospital from March 2022 to March 2024 were selected as the research subjects.They were divided into a control group and an observation group according to different treatment methods,with 50 patients in each group,the control group received conventional treatment,the observation group received fiberoptic bronchoscopy treatment.The respiratory dynamic indicators of the two groups,including airway resistance(Raw),peak airway pressure(PIP),and dynamic compliance(Cdyn),as well as inflammatory factors such as high-sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)levels,and procalcitonin(PCT)levels,were compared rehabilitation effect and clinical efficacy.Results After 14 days of treatment,the levels of Raw and PIP in both groups were lower than before treatment,and the level of Cdyn was higher than before treatment(P<0.05).The levels of Raw and PIP in the observation group were lower than those in the control group,and the level of Cdyn was higher than that in the control group(P<0.05);After 14 days of treatment,the levels of hs-CRP,TNF-α,and PCT in both groups were lower than before treatment(P<0.05).The levels of hs-CRP,TNF-α,and PCT in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05);The mechanical ventilation time,improvement time of pulmonary infection,and length of ICU stay in the observation group were significantly shorter than those in the control group(P<0.05);The total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion Fiberoptic bronchoscopy bronchoalveolar lavage can improve respiratory dynamics indicators,enhance lung function,inhibit inflammatory response,and promote patient recovery in ICU patients wit
