低分子肝素联合乙酰半胱氨酸、泼尼松治疗特发性间质性肺炎的效果分析

摘　　要：目的:分析低分子肝素联合乙酰半胱氨酸、泼尼松治疗特发性间质性肺炎的效果。方法:选择2021年1月—2024年1月于张家港澳洋医院呼吸内科就诊的78例特发性间质性肺炎患者进行研究,根据随机数字表法分为对照组(n=39)、试验组(n=39)。对照组给予泼尼松治疗,试验组在对照组基础上采用低分子肝素与乙酰半胱氨酸治疗。比较两组临床疗效。结果:试验组治疗总有效率高于对照组(P=0.023)。治疗后,两组血清涎液化糖链抗原-6、超敏C反应蛋白水平均降低,且试验组低于对照组(P<0.05)。治疗后,两组肺总量、一氧化碳弥散量占预计值百分比均升高,且试验组高于对照组(P<0.05)。治疗后,两组动脉血氧分压增高,动脉血二氧化碳分压降低,且试验组改善幅度大于对照组(P<0.05)。结论:低分子肝素联合乙酰半胱氨酸、泼尼松治疗特发性间质性肺炎的效果较好,可有效改善患者肺功能,纠正动脉血气紊乱,下调血清涎液化糖链抗原-6、超敏C反应蛋白表达量。Objective:To analyze the therapeutic effect of low molecular weight heparin combined with acetylcysteine and prednisone for idiopathic interstitial pneumonia.Methods:A total of 78 patients with idiopathic interstitial pneumonia who visited the Respiratory Medicine Department of Zhangjiagang Aoyang Hospital from January 2021 to January 2024 were selected for study.They were divided into a control group(n=39)and a test group(n=39)using a random number table method.The control group was treated with prednisone,and the test group was treated with low molecular weight heparin and acetylcysteine on the basis of control group.The clinical efficacy was compared between the two groups.Results:The total effective rate of treatment in the test group was higher than that in the control group(P=0.023).After treatment,the levels of serum salivary liquefied glycan antigen-6 and hypersensitive C-reactive protein in the two groups decreased,and the levels in the test group were lower than those in the control group(P<0.05).After treatment,the total lung capacity and the percentage of carbon monoxide diffusion capacity to the predicted value in the two groups increased,and these parameters in the test group were higher than those in the control group(P<0.05).After treatment,the arterial partial pressure of oxygen increased and the arterial partial pressure of carbon dioxide decreased in the two groups,and the improvement in the test group was greater than that in the control group(P<0.05).Conclusion:Low molecular weight heparin combined with acetylcysteine and prednisone has a good effect in the treatment of idiopathic interstitial pneumonia,and can effectively improve lung function,correct arterial blood gas disorders,and downregulate the expression of serum salivary liquefied glycan antigen-6 and hypersensitive C-reactive protein.
