关节镜下双通道“8”字捆绑技术治疗后交叉韧带胫骨止点撕脱骨折的效果分析

摘　　要：目的:分析关节镜下双通道“8”字捆绑技术治疗后交叉韧带胫骨止点撕脱骨折的效果。方法:选取2022年1月—2024年6月北京怀柔医院收治的交叉韧带胫骨止点撕脱骨折患者64例作为研究对象,采用随机数字表法分为对照组和研究组,各32例。对照组采用不可吸收缝线进行固定,研究组采用关节镜下双通道“8”字捆绑技术固定。比较两组手术指标、术后恢复情况、膝关节功能。结果:研究组手术时间短于对照组,术中出血量少于对照组(P<0.001)。研究组骨折愈合时间短于对照组,并发症总发生率低于对照组(P<0.05)。术前,两组膝关节功能评分对比,无统计学差异(P>0.05);术后2、4、6个月,两组膝关节功能评分逐渐升高,且研究组高于对照组(P<0.05)。结论:关节镜下双通道“8”字捆绑技术治疗后交叉韧带胫骨止点撕脱骨折的效果显著,可缩短手术时间,减少术中出血量,促进骨折愈合,加速膝关节功能恢复,降低并发症发生率。Objective:To analyze the effect of arthroscopic double-channel"8"bundling technique in treatment of tibial insertion avulsion fractures of posterior cruciate ligament.Methods:A total of 64 patients with tibial insertion avulsion fractures of posterior cruciate ligament admitted to Beijing Huairou Hospital from January 2022 to June 2024 were selected as the study subjects.They were divided into control group and study group according to random number table method,with 32 cases in each group.The con⁃trol group was treated with non-absorbable suture fixation and the study group was treated with arthroscopic double-channel"8"bundling technique fixation.Surgical indexes,postoperative recovery and knee joint function were compared between two groups.Results:The operation time in the study group was shorter than that in the control group,and the intraoperative blood loss in the study group was less than that in the control group(P<0.001).The fracture healing time in the study group was shorter than that in the control group,and the total incidence of complications in the study group was lower than that in the control group(P<0.05).Be⁃fore operation,there was no statistically significant difference in knee joint function scores between the two groups(P>0.05).At 2,4 and 6 months after operation,the knee joint function scores in the two groups gradually increased and the scores in the study group were higher than those in the control group(P<0.05).Conclusion:Arthroscopic double-channel"8"binding technique has a significant effect in the treatment of tibial insertion avulsion fractures of posterior cruciate ligament,and can shorten the operation time,reduce the intraoperative blood loss,promote fracture healing,accelerate the recovery of knee joint function,and reduce the incidence of complications.
