低温等离子射频扁桃体及腺样体切除术治疗儿童阻塞性睡眠呼吸暂停综合征的临床效果

摘  要:目的:分析低温等离子射频扁桃体及腺样体切除术治疗儿童阻塞性睡眠呼吸暂停综合征的临床效果。方法:选择2023年7月—2024年7月常熟市第一人民医院收治的80例阻塞性睡眠呼吸暂停综合征患儿进行研究,以抽签法分为参照组与研究组,每组40例。参照组实施传统腺样体扁桃体切除术,研究组实施低温等离子射频扁桃体及腺样体切除术。比较两组临床指标、治疗效果,术前、术后细胞免疫指标与体液免疫指标。结果:研究组治疗总有效率为97.50%,高于参照组(P<0.05)。与参照组比较,研究组术中出血量更少,手术时间更短,打鼾缓解时间更早(P<0.05)。术前、术后,两组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、免疫球蛋白A、免疫球蛋白G与免疫球蛋白M水平比较,差异均无统计学意义(P>0.05)。结论:低温等离子射频扁桃体及腺样体切除术、传统腺样体扁桃体切除术治疗儿童阻塞性睡眠呼吸暂停综合征,均不会对患儿的细胞免疫与体液免疫指标造成影响,但相比于传统腺样体扁桃体切除术,低温等离子射频扁桃体及腺样体切除术创伤小,能够减少术中出血量,促进患儿康复。Objective:To analyze the clinical effect of low-temperature plasma radio-frequency tonsillectomy and adenoidectomy in treatment of children with obstructive sleep apnea syndrome.Methods:From July 2023 to July 2024,80 children with obstruc⁃tive sleep apnea syndrome admitted to Changshu No.1 People's Hospital were selected for study.They were divided into a refer⁃ence group and a study group using the lottery method,with 40 cases in each group.The reference group received traditional ad⁃enotonsillectomy,and the study group received low-temperature plasma radio-frequency tonsillectomy and adenoidectomy.The clinical indexes,treatment effect,preoperative and postoperative cellular immunity indexes and humoral immunity indexes were compared between the two groups.Results:The total effective rate of treatment in the study group was 97.50%,which was higher than that in the reference group(P<0.05).Compared with the reference group,the study group had less intraoperative blood loss,shorter operation time,and earlier snoring relief time(P<0.05).There was no statistically significant difference in the levels of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),immunoglobulin A,immunoglobulin G and immunoglobulin M between the two groups before and after surgery(P>0.05).Conclusion:Low-temperature plasma radio-frequency tonsillectomy and adenoidectomy and traditional ad⁃enotonsillectomy in the treatment of children with obstructive sleep apnea syndrome will not affect the cellular immunity and hu⁃moral immunity indexes of children,but compared with traditional adenotonsillectomy,low-temperature plasma radio-frequency tonsillectomy and adenoidectomy are less invasive,and can reduce intraoperative blood loss and promote the children's recovery.
