预见性护理在留置经外周静脉穿刺中心静脉导管的肿瘤化疗患者中的应用效果分析

摘　　要：目的:分析预见性护理在留置经外周静脉穿刺中心静脉导管(PICC)的肿瘤化疗患者中的应用效果。方法:选取2022年7月—2024年7月锦州医科大学附属第三医院收治的留置PICC的肿瘤化疗患者88例作为研究对象,依据随机数字表法分为对照组和观察组,各44例。对照组实施常规护理,观察组实施预见性护理。比较两组护理效果。结果:护理后,两组视觉模拟评分法、现有疼痛强度、疼痛分级评分及总分降低,且观察组低于对照组(P<0.05)。护理后,两组腋下血流速度、平均速度高于护理前,且观察组高于对照组(P<0.05);两组凝血因子反应时间长于护理前,且观察组长于对照组(P<0.05);两组两侧曲线最宽距离小于护理前,且观察组小于对照组(P<0.05)。观察组PICC并发症总发生率低于对照组(P=0.035)。观察组护理总满意度高于对照组(P=0.002)。结论:预见性护理在留置PICC的肿瘤化疗患者中的应用效果显著,可减轻患者疼痛,改善血流动力学指标、血栓弹力图指标,降低PICC并发症发生率,提高护理满意度。Objective:To analyze the application effect of predictive nursing in tumor chemotherapy patients indwelled with pe⁃ripherally inserted central catheter(PICC).Methods:A total of 88 tumor chemotherapy patients indwelled with PICC who were ad⁃mitted to the Third Affiliated Hospital of Jinzhou Medical University from July 2022 to July 2024 were selected as the study sub⁃jects.They were divided into a control group and an observation group by using a random number table method,with 44 cases in each group.The control group received conventional nursing,and the observation group received predictive nursing.The nursing ef⁃fect was compared between the two groups.Results:After nursing,the scores of visual analogue scale,existing pain intensity,pain grading and total scores in the two groups reduced,and the scores in the observation group were lower than those in the control group(P<0.05).After nursing,the axillary blood flow velocity and average velocity in the two groups were higher than those before nursing,and these parameters in the observation group were higher than those in the control group(P<0.05).The reaction time of coagulation factors in the two groups was longer than that before nursing,and this parameter in the observation group was longer than that in the control group(P<0.05).The widest distance between bilateral curves in the two groups was smaller than that before nursing,and this parameter in the observation group was smaller than that in the control group(P<0.05).The total incidence of PICC complications in the observation group was lower than that in the control group(P=0.035).The total satisfaction rate with nursing in the observation group was higher than that in the control group(P=0.002).Conclusion:The application of predictive nursing in tumor chemotherapy patients indwelled with PICC has a significant effect,which can relieve patients'pain,improve he⁃modynamic and thromboelastography indicators,reduce the incidence of PICC complications,and increase nursing satisfaction.
