摘  要:目的通过疼痛行为学和静息态功能磁共振(Rs-fMRI)技术检测,探究推拿对神经病理性疼痛(NP)大鼠行为学表现以及疼痛相关脑区神经活动变化的影响,进一步探析推拿镇痛的可能机制。方法将24只雄性SD大鼠采用随机数表法分为3组,即空白组、模型组和推拿组,每组各8只。各组大鼠分别在造模前1天和造模后第4、10、14、17天进行机械痛阈值(PWT)检测。各组随机选取3只大鼠于造模后第18天进行RsfMRI扫描,计算局部一致性(ReHo)和低频振幅(ALFF)。结果模型组大鼠右后足PWT小于空白组PWT,推拿组大鼠右后足PWT大于模型组PWT,差异有统计学意义(P<0.001)。各组大鼠ReHo值存在显著差异的脑区有左侧次级运动皮层,ALFF值存在显著差异的脑区有左侧内嗅皮层和右侧初级躯体感觉皮层,差异有统计学意义(P<0.05)。结论推拿能够改善NP大鼠疼痛行为,其可能与调控内嗅皮层、次级运动皮层和初级躯体感觉皮层等疼痛相关脑区神经活动有关。Objective To investigate the effects of massage on the behavioral manifestations and pain related brain activity changes in neuropathic pain(NP)rats through pain behavior and resting state functional magnetic resonance imaging(Rs-fMRI)techniques,and further explore the possible mechanisms of massage analgesia.Methods A total of 24 male SD rats were randomly divided into three groups using a random number table,namely a blank group,a model group,and a massage group,with 8 rats in each group.Paw withdrawal threshold(PWT)testing was performed on each group of rats on the day before modeling and on the 4th,10th,14th,and 17th days after modeling.Three rats were randomly selected from each group for Rs-fMRI scanning on the 18th day after modeling,and regional homogeneity(ReHo)and amplitude of low frequency fluctuation(ALFF)were calculated.Results The PWT of the right hind foot of the model group rats was lower than that of the blank group,and the PWT of the right hind foot of the massage group rats was higher than that of the model group,with statistically significant difference(P<0.001).The brain regions with significant differences in ReHo values among the groups of rats included the left secondary motor cortex,while the brain regions with significant differences in ALFF values included the left entorhinal cortex and the right primary somatosensory cortex,and the differences were statistically significant(P<0.05).Conclusion Massage can improve the pain behavior of NP rats,which may be related to the regulation of nerve activities in pain related brain regions such as the olfactory cortex,the secondary motor cortex and the primary somatosensory cortex.
