温脾化瘀方结合麝香灸治疗胰腺癌临床效果观察

摘　　要：目的观察温脾化瘀方结合麝香灸治疗胰腺癌临床效果。方法纳入胰腺癌患者105例(2020年7月—2023年7月收治),以随机数字表法将患者分为两组,对照组52例采取临床对症治疗及化疗治疗,观察组53例在对照组治疗基础上结合温脾化瘀方以及麝香灸治疗,研究过程中对照组脱落1例,观察组脱落1例,观察两组数据:疗效、治疗前后中医证候积分(腹部包块、肢体畏冷、食少纳呆、肢体水肿、腹部鼓胀等)变化、癌胚抗原与糖类抗原199(CA199)水平变化、白介素-6(IL-6)与白介素-10(IL-10)、肿瘤坏死因子α(TNF-α)等炎症因子水平变化、T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)水平变化、不良反应、治疗前后患者卡氏评分(KPS)变化。结果观察组治疗效果优于对照组,P<0.05;治疗前,两组患者中医证候积分(腹部包块、肢体畏冷、食少纳呆、肢体水肿、腹部鼓胀等)、癌胚抗原与CA199、IL-6及IL-10、TNF-α水平、CD3^(+)与CD4^(+)、CD8^(+)水平、KPS评分比较,P>0.05,治疗后两组患者中医证候积分(腹部包块、肢体畏冷、食少纳呆、肢体水肿、腹部鼓胀等)、癌胚抗原与CA199、IL-6及IL-10、TNF-α与CD3^(+)与CD4^(+)、CD8^(+)水平均降低,KPS评分均上升,治疗后,观察组患者中医证候积分(腹部包块、肢体畏冷、食少纳呆、肢体水肿、腹部鼓胀等)、癌胚抗原与CA199、IL-6及IL-10、TNF-α水平比对照组更低,CD3^(+)及CD4^(+)、CD8^(+)水平与KPS评分均比对照组高,P<0.05;观察组的不良反应率比对照组患者更低,P<0.05。结论脾化瘀方结合麝香灸治疗胰腺癌临床效果甚佳,可较好抑制肿瘤细胞,改善患者症状,抑制机体炎症反应,提升机体免疫能力及生活质量,还可较好缓解化疗不良反应,保证治疗安全,值得应用。Objective To observe the clinical effect of Wenpi Huayu Recipe(温脾化瘀方)combined with musk moxibustion on pancreatic cancer.Methods In this study,a total of 105 patients with pancreatic cancer(admitted from July 2020 to July 2023)were divided into two groups by random number table method.The patients in the control group(52 cases)received clinical symptomatic treatment and chemotherapy and those in the observation group(53 cases)received combined treatment with Wenpi Huayu Recipe and musk moxibustion on the basis of the treatment in the control group.During the study,1 case fell off in the control group and 1 case in the observation group.The data of each group was observed,including curative effect,TCM syndrome scores(abdominal lumps,limb coldness,overeating,limb edema,abdominal distension,etc),the levels of carcinoembryonic antigen and carbohydrate antigen 199(CA199)before and after treatment,the levels of interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factorα(TNF-α)and other inflammatory factors,the levels of T lymphocyte subsets CD3^(+),CD4^(+),CD8^(+),adverse reactions and changes of Karnofsky performance status(KPS)before and after treatment.Results The treatment effect of the observation group was better than that of the control group(P<0.05).Before treatment,TCM syndrome scores(abdominal lumps,limb coldness,overeating,limb edema,abdominal distension,etc),the levels of CA199,IL-6,IL-10,TNF-α,CD3^(+),CD4^(+)and CD8^(+)and KPS scores were not significant in both groups(P>0.05).After treatment,TCM syndrome scores(abdominal lumps,limb coldness,overeating,limb edema,abdominal distension,etc),levels of carcinoembryonic antigen and CA199,IL-6 and IL-10,TNF-αand CD3^(+),CD4^(+)and CD8^(+)of patients in all groups were decreased,and KPS scores were increased.Traditional Chinese medicine syndrome scores(abdominal mass,limb chills,lack of food,limb edema,abdominal distension,etc.),levels of carcinoembryonic antigen,CA199,IL-6,IL-10 and TNF-αin observation group were lower than those in control g
