电针针刺膀胱募穴穴组治疗女性压力性尿失禁的临床研究

摘　　要：目的观察电针针刺膀胱募穴穴组对轻、中度女性压力性尿失禁(SUI)的临床疗效。方法将80例SUI患者按1∶1比例随机分为观察组、对照组,每组40例。在盆底肌训练(PFMT)基础上,观察组取中极、关元、大赫(双侧)穴进行电针治疗,对照组取穴同观察组,施以安慰电针治疗;两组治疗均为每周3次,疗程6周。比较两组1h尿垫试验漏尿量、国际尿失禁咨询委员会尿失禁问卷简表(ICI-Q-SF)评分、平均24h尿失禁次数、漏尿程度变化情况以及主观疗效和安全性评价。结果①最终完成试验者73例,观察组37例、对照组36例。②治疗2、4、6周后与治疗前组内比较,观察组1h尿垫试验漏尿量均降低(P<0.05),对照组在治疗4、6周时降低(P<0.05);组间治疗2、4、6周后比较,观察组1h尿垫试验漏尿量均低于对照组(P<0.05)。③治疗2、4、6周后与治疗前组内比较,观察组平均24h尿失禁次数均减少(P<0.05),对照组在治疗4、6周时减少(P<0.05);组间治疗2、4、6周后比较,观察组平均24h尿失禁次数均低于对照组(P<0.05)。④治疗6周后,两组ICI-Q-SF总分、漏尿程度较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05)。⑤治疗过程中,两组均未出现明显不良反应。结论在以PFMT为基础治疗的情况下,电针针刺膀胱募穴穴组能降低SUI患者漏尿量、漏尿程度,患者主观疗效评价高,可见本治疗方案安全有效。Objective To observe the clinical efficacy of electroacupuncture at bladder Mu point group in treating mild to moderate female stress urinary incontinence(SUI).Methods This trial included 80 patients with SUI,which randomly divided into an observation group(n=40)and a control group(n=40)in a 1∶1 ratio.Both groups underwent Pelvic Floor Muscle Training(PFMT).The observation group received electroacupuncture at Zhongji(CV3),Guanyuan(CV4),and bilateral Dahe(KI12)points,three times a week for six weeks.The control group received sham electroacupuncture at the same points with the same frequency and duration.Outcomes measured and compared between two groups were 1-hour urine pad test leakage volume,International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form(ICI-Q-SF)score,average 24-hour incontinence episodes,urine leakage severity,subjective efficacy,and safety.Results①Finally,73 patients completed the trial,with 37 in the observation group and 36 in the control group.②After 2,4,and 6 weeks of treatment,the 1-hour urine pad test leakage volume in the observation group was significantly reduced compared to baseline(P<0.05).In the control group,the reduction was significant at 4 and 6 weeks(P<0.05).When comparing the two groups at 2,4,and 6 weeks,the observation group had significantly lower 1-hour urine pad test leakage volumes than the control group(P<0.05).③After 2,4,and 6 weeks of treatment,the average 24-hour incontinence episodes in the observation group significantly decreased compared to baseline(P<0.05).In the control group,the decrease was significant at 4 and 6 weeks(P<0.05).When comparing the two groups at 2,4,and 6 weeks,the observation group had significantly fewer average 24-hour incontinence episodes than the control group(P<0.05).④After 6 weeks of treatment,both groups showed a significant reduction in ICI-Q-SF total scores and urine leakage severity compared to baseline(P<0.05),with the observation group showing greater improvements than the control group(P<0.05)
