基于眼电信号关联维数特征探讨针灸“治神”对得气针感的影响

摘　　要：目的基于眼电信号关联维数(D2)特征探讨得气针感在不同关注状态下针刺的表现形式、关注状态与得气针感强度的关系等,以期为针刺“治神”与得气理论提供科学依据。方法50例健康受试者以相同顺序依次接受3种关注状态下的足三里穴针刺,每次干预间隔7d洗脱期。运用自主研发装置和中文改进版马萨诸塞医院针刺感觉量表(C-MMASS)采集眼电信号和得气针感,比较3种关注状态下得气针感强度、眼电信号D2以及二者间的相关性,利用受试者操作特征(ROC)曲线评估D2对得气结局的预测价值。结果①关注状态Ⅰ的马萨诸塞医院针刺感觉量表(MASS)指数、酸感、胀感显著高于关注状态Ⅱ(P<0.05)及关注状态Ⅲ(P<0.05),跳动感及其他针感在关注状态Ⅰ中显著高于关注状态Ⅲ(P<0.05)。轮廓分析显示,酸感、麻感、胀感、沉重感在3种关注状态下总体轮廓相互平行(F=0.916,P=0.484>0.05),不同状态间轮廓没有重合(F=5.804,P<0.01);痛感、压迫感、钝痛感、刺感在3种关注状态下总体轮廓相互平行(F=0.608,P=0.484>0.05),不同状态间轮廓重合(F=10.812,P>0.05)。②关注状态Ⅰ中D2显著低于关注状态Ⅱ(P<0.05)及关注状态Ⅲ(P<0.05),同时关注状态Ⅱ中D2也显著低于关注状态Ⅲ(P<0.05)。③相关性分析显示,酸感(P<0.01,r=-0.31)、刺感(P<0.05,r=-0.19)、麻感(P<0.05,r=-0.17)分别与D2存在低等程度负相关关系;胀感(P<0.01,r=-0.43)、MASS指数(P<0.01,r=-0.41)分别与D2呈现中等程度负相关关系。④D2预测得气结局的ROC曲线下面积(AUC)为0.804,D2=1.90为预测得气的临界值,灵敏度为85.2%,特异性为64.3%。结论得气针感的提高可能与受试者注意力专注于针刺全过程的“治神”状态有关;而采用眼电信号D2特征对得气结局进行预测具有可行性及较好的预测性能,有望成为临床医者研判“得气”的新方法。Objective To explore the manifestations of Deqi needling sensation under different attention states,and the relationship between attention states and the intensity of Deqi based on the correlation dimension(D2)characteristics of electrooculogram(EOG)signals,in order to provide scientific evidence for the theories of"treating the spirit"and Deqi in acupuncture.Methods In this study,50 healthy participants underwent acupuncture at the Zusanli(ST36)point under three different attention states,with a 7-day washout period between each intervention.EOG signals and Deqi sensations were recorded using a self-developed device and the Chinese modified version of the Massachusetts General Hospital Acupuncture Sensation Scale(C-MMASS).The intensity of the Deqi sensations and the EOG signals D2 under the three attention states,and correlation analysis between the two were compared.The ROC curve was used to evaluate the predictive value of D2 for the outcome of Deqi.Results①The MASS index,soreness,and fullness sensations in attention stateⅠwere significantly higher than those in stateⅡ(P<0.05)and stateⅢ(P<0.05).Throbbing and other needling sensations were significantly higher in attention state I than in stateⅢ(P<0.05).The contour analysis showed that the sensations of soreness,numbness,fullness,and heaviness exhibited parallel contours across the three attention states(F=0.916,P=0.484>0.05),with no overlap between states(F=5.804,P<0.01).Pain,pressure,dull pain,and tingling sensations also showed parallel contours across the three attention states(F=0.608,P=0.484>0.05),with overlapping contours between states(F=10.812,P>0.05).②In attention stateⅠ,D2 level was significantly lower than that in stateⅡ(P<0.05)and stateⅢ(P<0.05),and D2 level in attention stateⅡwas also significantly lower than that in stateⅢ(P<0.05).③Correlation analysis showed a weak negative correlation between D2 and sensations of soreness(P<0.01,r=-0.31),tingling(P<0.05,r=-0.19),and numbness(P<0.05,r=-0.17).A moderate negative correlatio
