摘 要:目的:探讨骨水泥分布形态对经皮椎体成形术(PVP)后疗效的影响。方法:回顾性分析80例2020年1月至2022年6月在我院骨科采用PVP治疗老年骨质疏松性椎体压缩性骨折患者的临床资料,根据术后伤椎内骨水泥分布不同形态将患者分为团块状组(n=34)和海绵状组(n=46)。比较两组不同时段VAS评分、ODI评分、伤椎前缘高度、伤椎及相邻椎体再次骨折例数、伤椎相邻节段平均椎间盘高度及伤椎区域Cobb角变化。结果:末次随访时,团块状组VAS、ODI评分高于海绵状组,伤椎前缘高度、伤椎上下椎间盘平均高度低于海绵状组,伤椎区域后凸Cobb角大于海绵状组(均P<0.05)。结论:PVP治疗老年骨质疏松性椎体压缩性骨折术后,早期不同骨水泥分布形态均可获得较好疗效,但中后期骨水泥呈海绵状分布的疗效优于团块状分布。Objective:To investigate the effect of bone cement distribution morphology on the efficacy of percutaneous vertebroplasty(PVP).Methods:The clinical data of 80 patients with senile osteoporotic vertebral compression fractures who underwent PVP in the Department of Orthopedics of our hospital from January 2020 to June 2022 were retrospectively analyzed.The patients were divided into a massive group(n=34)and a spongy group(n=46)according to the different distribution morphologies of bone cement in the fractured vertebrae after surgery.The VAS scores,ODI scores,anterior height of the fractured vertebrae,number of re-fractures of the fractured vertebrae and adjacent vertebrae,average intervertebral disc height of the adjacent segments of the fractured vertebrae,and changes in the Cobb angle of the fractured vertebrae area were compared between the two groups at different time periods.Results:At the last follow-up,the VAS and ODI scores of the massive group were higher than those of the spongy group,the anterior height of the fractured vertebrae and the average height of the upper and lower intervertebral discs of the fractured vertebrae were lower than those of the spongy group,and the kyphotic Cobb angle of the fractured vertebrae area was larger than that of the spongy group(all P<0.05).Conclusion:After PVP treatment for senile osteoporotic vertebral compression fractures,different bone cement distribution morphologies can achieve good efficacy in the early stage,but the efficacy of the spongy distribution of bone cement is better than that of the massive distribution in the middle and late stages.
