某医院2022-2023年医院感染现患率分析

摘　　要：目的了解本院的医院感染现患情况。方法采用横断面调查方法,选取2022年10月19日-2023年9月1日调查日住院患者,了解医院感染现患率、科室分布、病原体、抗生素等情况。结果2年的调查日,共调查4023例患者,医院感染134例,医院感染现患率2022年为3.98%、2021年为2.68%,2023年较2022年医院感染现患率下降,差异有统计学意义(P<0.05)。ICU病房医院感染现患率最高,儿科、五官科无院感病例,外科医院感染现患率下降,差异有统计学意义(P<0.05)。病原体以革兰阴性菌占比最高,感染部位下呼吸道感染为主,其次是泌尿道感染。抗菌药物使用率呈现上升趋势,差异有统计学意义(P<0.05),治疗前已送细菌培养率差异无统计学意义(P>0.05)。结论今后的任务是要继续加强住院患者抗生素使用管理,加强对重点科室、重要病原体医院感染防控及监测,更加科学规范医院感染管理。Objective To understand the prevalence of nosocomial infection in our hospital.Methods A cross-sectional survey was used to investigate the prevalence of nosocomial infection among hospitalized patients from Oct.19,2022 to Sep.1,2023 to understand the prevalence rate of nosocomial infection,department distribution,infection pathogen,antibiotics,etc.Results A total of 4023 patients were investigated in the past two years,including 134 cases of nosocomial infection.The prevalence rates of nosocomial infection was 3.98%in 2022 and 2.68%in 2021.The incidence rate of nosocomial infection in 2023 was lower than that in 2022,and the difference was statistically significant(P<0.05).Gram-negative bacteria accounted for the highest proportion of pathogens,and the main infection site was lower respiratory tract infection,followed by urinary tract infection.The utilization rate of antibiotics showed an increasing trend,with statistical significance(P<0.05);There was no significant difference in the culture rate of delivered bacteria before treatment(P>0.05).Conclusions The future task is to continue to strengthen the management of antibiotic use in inpatients,strengthen the prevention,control and monitoring of nosocomial infection in key departments and important pathogens,and more scientifically standardize the management of nosocomial infection.
