子宫内膜异位症保守手术后地诺孕素与促性腺激素释放激素激动剂的效果比较

摘　　要：目的探讨腹腔镜保守治疗行手术联合常用药物对子宫内膜异位症(EMS)患者的治疗效果,以期明确何种药物是预防EMS复发的有效术后疗法。方法选取101例行腹腔镜保守性手术治疗的EMS患者,按照术后联合应用不同药物分为A组(36例,术后联合应用地诺孕素治疗)、B组[34例,术后联合应用促性腺激素释放激素激动剂(GnRH-a)治疗]、C组(31例,单纯行腹腔镜保守性手术),腹腔镜手术后随访患者12个月,比较三组治疗效果。结果A组复发率为8.3%,低于C组的54.8%(P<0.05)。治疗后,A组、B组疼痛评分均低于C组(P<0.05)。A组不规则阴道出血发生率高于B组(P<0.05)。B组潮热发生率高于A组(P<0.05)。结论地诺孕素可用于EMS患者6个月以上的长期给药,相比GnRH-a药物治疗更有效,Gn RH-a药物治疗仅用于短期给药。Objective To explore the therapeutic effect of laparoscopic conservative treatment combined with commonly used drugs on patients with endometriosis(EMS),in order to clarify which drugs are effective postoperative therapies for preventing EMS recurrence.Methods 101 cases of EMS patients undergoing laparoscopic conservative surgery were selected and divided into Group A(36 cases,treated with Denogestrel after surgery),Group B[34 cases,treated with gonadotropin-releasing hormone agonist(GnRH-a)after surgery],and Group C(31 cases,treated with laparoscopic conservative surgery alone).Patients were followed up for 12 months after laparoscopic surgery to compare the treatment effects of the three groups.Results The recurrence rate of Group A was 8.3% lower than that of Group C,which was 54.8%(P<0.05).After treatment,the pain scores of Group A and Group B were lower than those of Group C(P<0.05).The incidence of irregular vaginal bleeding in Group A was higher than that in Group B(P<0.05).The incidence of hot flashes in Group B was higher than that in Group A(P<0.05).Conclusion Denogestrel can be used for long-term administration in EMS patients for more than 6 months,and is more effective than GnRH-a drug therapy,which is only used for short-term administration.
