摘　　要：目的观察经皮加压钢板与防旋股骨近端髓内钉固定治疗股骨近端骨折的临床疗效。方法选取2021年11月-2023年10月会昌县人民医院收治的60例股骨近端骨折患者为研究对象,采用随机数字表法分为对照组(n=30)和研究组(n=30)。对照组采用经皮加压钢板治疗,研究组采用防旋股骨近端髓内钉固定治疗,比较两组治疗效果、临床手术指标、髋关节功能、并发症发生率。结果研究组治疗总有效率(96.67%)优于对照组(73.33%)(P<0.05);与对照组比较,研究组手术时间、术中出血量、负重行走时间、骨折愈合时间、卧床时间、住院时间均较短(P<0.05);与对照组比较,研究组治疗后功能、活动度、畸形、疼痛、总评分等髋关节功能评分均较高(P<0.05);研究组并发症发生率(3.33%)低于对照组(33.33%)(P<0.05)。结论防旋股骨近端髓内钉固定治疗股骨近端骨折的临床疗效确切,能够有效改善患者髋关节功能,减轻患者手术创伤,提高患者术后康复效果,且安全性较高,值得临床应用。Objective To observe the clinical efficacy of percutaneous compression plate and anti-rotation proximal femoral intramedullary nail fixation in the treatment of proximal femoral fractures.Methods A total of 60 patients with proximal femoral fractures admitted to Huichang County People′s Hospital from November 2021 to October 2023 were selected as the research objects.They were divided into control group and study group by random number table method,with 30 patients in each group.The control group was treated with percutaneous compression plate,and the study group was treated with anti-rotation proximal femoral intramedullary nail fixation.The therapeutic effect,clinical surgical indicators,hip function and complication rate were compared between the two groups.Results The total effective rate of treatment in the study group(96.67%)was better than that in the control group(73.33%)(P<0.05).Compared with the control group,the operation time,intraoperative blood loss,weight-bearing walking time,fracture healing time,bed rest time and hospitalization time of the study group were shorter(P<0.05).Compared with the control group,the scores of hip joint function such as function,activity,deformity,pain and total score after treatment in the study group were higher(P<0.05).The incidence of complications in the study group(3.33%)was lower than that in the control group(33.33%)(P<0.05).Conclusion The clinical efficacy of anti-rotation proximal femoral intramedullary nail fixation in the treatment of proximal femoral fractures is definite,which can effectively improve the hip function of patients,reduce the surgical trauma of patients,improve the postoperative rehabilitation effect of patients,and has high safety.It is worthy of clinical application.
