慢性活动性EB病毒感染并EB病毒相关噬血细胞综合征1例

摘　　要：分析1例青年慢性活动性EB病毒感染合并EB病毒相关嗜血细胞综合征病例的诊疗过程。患者无风湿、肿瘤、免疫缺陷及药物过敏史,以高热、皮疹伴颈淋巴结肿大为首发症状,症状反复发作且不断进展,辅助检查显示患者脾大、血细胞减少、铁蛋白升高、可溶性白介素-2受体升高且淋巴结活检EBER原位杂交阳性,诊断为慢性活动性EB病毒感染合并EB病毒相关嗜血细胞综合征。经糖皮质激素、丙种球蛋白、更昔洛韦和膦甲酸钠治疗,患者最终转归良好。To analyze the diagnosis and treatment process of a young patient with chronic active EB virus infection combined with EB virus related hemophilic cell syndrome.The patient had no history of rheumatism,tumor,immune deficiency,or drug allergy.The initial symptoms were high fever,rash,and cervical lymph node enlargement,which occurred repeatedly and progressed continuously.Auxiliary examination showed splenomegalia,hemocytopenia,elevated ferritin,elevated soluble interleukin-2 receptor,and EBER in situ hybridization in lymph node biopsy was positive.The diagnosis was chronic active EB virus infection combined with EB virus related hemophilic cell syndrome.After treatment with corticosteroids,immunoglobulin,ganciclovir,and foscarnet sodium,the patient ultimately had a good outcome.
