黄体酮联合炔雌醇环丙孕酮治疗无排卵性异常子宫出血患者的疗效观察

摘　　要：目的观察黄体酮联合炔雌醇环丙孕酮治疗无排卵性异常子宫出血(AUB-O)患者的临床疗效。方法选取2022年1月至2024年2月收治的82例AUB-O患者,采用随机数表分为对照组(n=41)和观察组(n=41)。两组均接受诊断性刮宫,对照组接受炔雌醇环丙孕酮片治疗,观察组在此基础上联合黄体酮治疗。比较两组的治疗效果,并在治疗前后比较两组的月经量、月经期、子宫内膜厚度及炎症因子水平[白介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)],同时比较两组不良反应的发生率。结果治疗后,与对照组比较,观察组总有效率更高(P<0.05);两组月经量减少,经期缩短,子宫内膜厚度降低(P<0.05),且观察组月经量减少,经期缩短,子宫内膜厚度改善较对照组更明显(P<0.01);两组TNF-α和IL-2水平均下降(P<0.05),且观察组TNF-α和IL-2下降幅度较对照组更明显(P<0.001)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论黄体酮联合炔雌醇环丙孕酮治疗AUB-O患者,可减少月经量,调节月经期,减轻炎症反应。Objective To observe the effects of progesterone combined with ethinylestradiol and cyproterone tablets on inflammatory factors and menstrual flow in patients with abnormal uterine bleeding-ovulatory dysfunction(AUB-O).Methods 82 patients with AUB-O admitted to hospital from January 2022 to February 2024 were selected,and they were divided into control group(n=41)and observation group(n=41)by random number table.Both groups were given dilatation and curettage,the control group were treated with ethinylestradiol and cyproterone acetate tablets,and the observation group were given combined treatment with progesterone on this basis.And then,the therapeutic effects of the two groups,and the menstrual flow,menstrual period,endometrial thickness,and the levels of inflammatory factors(interleukin-2[IL-2],tumor necrosis factor-α[TNF-α])before and after treatment,as well as the incidence of adverse reactions between the two groups were compared.Results After treatment,compared with the control group,the observation group had a higher overall effective rate(P<0.05).Menstrual blood volume decreased,menstrual period shortened,and endometrial thickness decreased in the two groups(P<0.05),and the observation group showed a more significant improvement in menstrual blood volume reduction,shortened menstrual periods and endometrial thickness compared to the control group(P<0.01).Both groups showed a decrease in TNF-αand IL-2 levels(P<0.05),and the observation group showed a more significant decrease in TNF-αand IL-2 levels compared to the control group(P<0.001).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Progesterone combined with ethinylestradiol and cyproterone can reduce menstrual blood volume,regulate menstrual periods,and alleviate inflammatory reactions in patients with AUB-O.
