二术二陈汤联合格列美脲治疗腹型肥胖2型糖尿病患者的疗效观察

摘　　要：目的探讨二术二陈汤联合格列美脲治疗腹型肥胖2型糖尿病(T2DM)患者疗效观察。方法选取2022年5月至2024年1月于昆山市第二人民医院治疗的72例腹型肥胖T2DM患者,采用随机数字表法分为西药组和联用组,每组36例。西药组采用格列美脲,联用组在西药组基础上加用二术二陈汤治疗。比较2组治疗疗效、中医证候积分、体重指数(BMI)、糖代谢指标、脂代谢指标及不良反应发生率。结果联用组患者的总有效率为94.44%,明显高于西药组的77.78%(P<0.05);治疗后,2组各项中医证候评分、BMI、内脏脂肪面积、空腹血糖(FBG)、餐后2 h血糖(2hPBG)、糖化血红蛋白(HbAlc)、三酰甘油(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)水平显著降低,且联用组显著低于西药组(P<0.05);西药组、联用组总不良反应发生率分别为13.89%、11.11%,组间比较差异无统计学意义(P>0.05)。结论二术二陈汤联合格列美脲治疗腹型肥胖T2DM患者,能产生较好的临床疗效,有效改善患者中医证候,抑制糖脂代谢水平,安全性较高。Objective To observe the efficacy of Erzhu Erchen Decoction combined with glimepiride on type 2 diabetes mellitus(T2DM)patients with abdominal obesity.Methods Seventy-two T2DM patients with abdominal obesity treated in Kunshan Second People's Hospital from May 2022 to January 2024 were selected.They were randomly separated into Western medicine group and combination group using a randomized numerical table method,with 36 cases in each group.Patients in both groups were treated with glimepiride,and those in the combination group were additionally treated with Erzhu Erchen Decoction.The treatment efficacy,traditional Chinese medicine(TCM)syndrome scores,body mass index(BMI),glucose metabolism index,lipid metabolism indicators and adverse events were compared between the two groups.Results The total effective rate was significantly higher in the combination group than that of the Western medicine group(94.44%vs 77.78%,P<0.05).After treatment,TCM syndrome scores,BMI,visceral fat area,fasting blood glucose(FBG),2h postprandial blood glucose(2hPBG),glycosylated hemoglobin(HbAlc),triglyceride(TG),total cholesterol(TC),and low-density lipoprotein cholesterol(LDL-C)of the two groups were significantly reduced,which were significantly lower in the combination group than the Western medicine group(P<0.05).The incidence of total adverse events was 13.89%in the Western medicine group and 11.11%in the combination group,showing no significant difference between the groups(P>0.05).Conclusion Erzhu Erchen Decoction combined with glimepiride for treating T2DM patients with abdominal obesity can produce a high clinical efficacy that effectively improves TCM syndromes,and inhibits the glucose and lipid metabolism with a high safety.
