摘 要:目的研究利多卡因气雾剂联合Valsalva呼吸减轻静脉输液港无损伤针穿刺疼痛的临床效果。方法选取2022年我院行静脉输液港无损伤穿刺的120例乳腺癌化疗治疗者为研究对象,按镇痛方法不同将其随机分为对照组及试验组,每组60例。比较2组患者的疼痛程度、心理状况、满意度、一次穿刺成功率、生命体征变化、心理状况及不良反应发生情况。结果与干预前比较,2组数学疼痛评分及汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分下降,且试验组疼痛数字评分及MAMA、HAMD评分低于对照组(P<0.05)。试验组患者一次穿刺成功率及满意度高于对照组(P<0.05)。2组干预前后心率、血氧饱和度、平均动脉压等变化比较,差异无统计学意义(P>0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论利多卡因气雾剂联合Valsalva呼吸能够有效改善静脉输液港无损伤针穿刺患者的疼痛情况及心理状况,有助于一次穿刺成功率及满意度的提高,不会对心率、血氧饱和度及平均动脉压等造成影响,也不会增加不良反应,安全性较高。Objective To study the clinical effect of lidocaine aerosol combined with Valsalva respiration on reducing pain during non-invasive needle puncture at the intravenous infusion port.Methods A total of 120 patients with breast cancer chemotherapy who underwent non-invasive puncture of intravenous infusion port in our hospital in 2022 were selected as the research objects.According to the different analgesic methods,they were randomly divided into the control group and the experimental group,with 60 cases in each group.The degree of pain,psychological status,satisfaction,success rate of first puncture,changes in vital signs,psychological status and adverse reactions were compared between the two groups.Results Compared with those before intervention,the mathematical pain score,the HAMA,and the HAMD in both groups were significantly decreased,which were significantly lower in the experimental group than the control group(P<0.05).The success rate and satisfaction rate of the first puncture in the experimental group were significantly higher than those of the control group(P<0.05).There were no significant differences in the vital signs such as heart rate,blood oxygen saturation,and mean arterial pressure before and after intervention between groups(P>0.05).There was no significant difference in the incidence of adverse events between the experimental group and the control group(P>0.05).Conclusion Lidocaine aerosol combined with Valsalva breathing can effectively alleviate the pain and psychological status of patients undergoing non-invasive needle puncture at the intravenous infusion port.It helps to increase the success rate of a single puncture and patient satisfaction without affecting vital signs such as heart rate,blood oxygen saturation,and mean arterial pressure,nor increasing adverse events.The safety is relatively high.
