恐惧疾病进展在乳腺癌患者压力知觉与抑郁间的中介效应

摘　　要：目的:探讨恐惧疾病进展在乳腺癌患者压力知觉与抑郁间的中介效应。方法:采用便利抽样,于2023年12月至2024年3月抽取湖南省某三甲医院乳腺肿瘤科住院的273例乳腺癌患者。使用一般资料调查问卷、汉密顿抑郁量表、中文版压力知觉量表及恐惧疾病进展量表进行问卷调查。构建并检验中介模型。结果:乳腺癌患者的抑郁得分6.53±4.61分,压力知觉得分为20.87±6.08分,恐惧疾病进展得分为30.64±8.11分。抑郁与压力知觉、恐惧疾病进展呈正相关(r=0.344,0.319;P<0.01)。恐惧疾病进展在压力知觉与抑郁之间起到部分中介作用,中介效应占总效应22.2%。结论:乳腺癌患者抑郁得分和压力知觉得分处于较低水平,恐惧疾病进展得分处于中等水平。乳腺癌患者压力知觉可通过恐惧疾病进展影响抑郁。Objective:To investigate the mediating effect of fear disease progression on stress perception and depression in breast cancer patients.Methods:From December 2023 to March 2024,273 patients with breast cancer who were hospitalized in the breast oncology department of a Grade-A hospital in Hunan Province were selected by convenience sampling.General Information Questionnaire,Hamilton Depression Scale,Chinese version of Stress Perception Scale and FearDisease Progression Scalewere used to conduct questionnaire survey.Build and test the mediation model.Results:Depression score was 6.24±4.49,stress perception score was 20.87±6.08 and fear of disease progression score were 30.65±8.11 in patients with breast cancer.Depression was positively correlated with stress perception and fear disease progression(r=0.344,0.319;P<0.01).Fear of disease progression played a partial mediating role between stress perception and depression,and the mediating effect accounted for 22.2%of the total effect.Conclusion:The depression and stress perception scores of breast cancer patients are low,and the fear of disease progression scores is medium.Stress perception in breast cancer patients may influence depression through fear of disease progression.
