口腔锥形束CT三维重建联合重组技术对上颌埋伏牙的诊断价值

摘  要:目的分析口腔锥形束CT三维重建联合重组技术对上颌埋伏牙的诊断价值。方法回顾性分析2019年12月—2021年6月期间邯郸市眼科医院(邯郸市第三医院)收治的96例疑有上颌埋伏牙(125颗牙)患者的临床资料,依据是否为埋伏牙分为埋伏牙组(n=72例,89颗牙)、非埋伏牙组(n=24例,36颗牙),均行锥形束CT(CBCT)三维重建及重组技术,比较两组的CBCT指标,分析上颌埋伏牙的影响因素、CT检查对上颌埋伏牙的诊断价值。结果埋伏牙组萌出角度大于非埋伏牙组,牙齿形态基本正常占比高于非埋伏牙组,患牙与邻牙分界清晰占比低于非埋伏牙组,患牙与邻牙距离短于非埋伏牙组(P<0.05),组间萌出方向比较P>0.05。多因素Logistic回归模型,可得出:萌出角度大(OR值:1.138,95%CI:1.054~1.229)、牙齿形态基本正常(OR值:0.128,95%CI:0.044~0.374)、患牙与邻牙的关系为邻牙推移(OR值:3.584,95%CI:1.598~8.040)、患牙与邻牙距离短(OR值:0.005,95%CI:0.000~0.077)为埋伏牙的独立危险因素(P<0.05)。CT检查诊断上颌埋伏牙的敏感度为95.50%,特异度为53.10%,AUC值为0.926。结论口腔CBCT三维重建联合重组技术对上颌埋伏牙具有较高的诊断价值。Objective To analyze the diagnostic value of conical beam CT 3D reconstruction combined with recombi-nation technique in maxillary embedded teeth.Methods The clinical data of 96 patients with suspected maxillary em-bedded teeth(125 teeth)admitted to our hospital from December,2019 to June,2021 were retrospectively analyzed.Ac-cording to whether the teeth were embedded,they were divided into the embedded teeth group(n=72 cases,89 teeth)and the non-embedded teeth group(n=24 cases,36 teeth).Cone beam CT(CBCT)3D reconstruction and recombination technology were performed in all patients.The CBCT indexes of the two groups were compared to analyze the influen-cing factors and the diagnostic value of CT for maxillary embedded teeth.Results The Angle of eruption in the ambush group was higher than that in the non-ambush group,the proportion of basically normal tooth shape was higher than that in the non-ambush group,the proportion of clear boundary between the affected and adjacent teeth was lower than that in the non-ambush group,the distance between the affected and adjacent teeth was shorter than that in the non-am-bush group(P<0.05),and the comparison of eruption direction between the groups was P>0.05.Multivariate Logis-tic regression model showed that the eruption Angle was large(OR value:1.138,95%CI:1.054~1.229),the tooth morphology was basically normal(OR value:0.128,95%CI:0.044~0.374),and the relationship between the affected tooth and the neighboring tooth was neighboring tooth displacement(OR value:3.584,95%CI:1.598~8.040),and short distance between affected and adjacent teeth(OR value:0.005,95%CI:0.000~0.077)were independent risk fac-tors for ambushing teeth(P<0.05).The sensitivity,specificity and AUC value of CT were 95.50%,53.10%and 0.926 respectively.Conclusion Oral CBCT 3D reconstruction combined with recombination technique has high diagnostic val-ue for maxillary embedded teeth.
