摘 要:目的探讨脂蛋白(a)水平与2型糖尿病患者胰岛素抵抗之间的相关性,揭示其在糖尿病代谢调控中的潜在作用。方法采用单中心横断面研究设计,纳入106例2型糖尿病患者,根据脂蛋白(a)水平分为高水平组(脂蛋白(a)>100mg/dL)与低水平组(脂蛋白(a)≤100 mg/dL)。收集一般信息及实验室指标,包括脂蛋白(a)、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)等,及计算胰岛素抵抗稳态模型评估指数(Homeo-stasis Model Assessment of Insulin Resistance,HOMA-IR)。通过Spearman相关分析评估脂蛋白(a)与HOMA-IR的相关性,采用多元线性回归模型控制混杂因素后验证两者的独立关系。结果高水平脂蛋白(a)组的HOMA-IR显著低于低水平组(P=0.036)。相关性分析显示,脂蛋白(a)水平与HOMA-IR呈负相关(r=-0.26,P=0.0066)。回归分析表明,调整年龄、性别、身体质量指数(BodyMassIndex,BMI)等混杂因素后,脂蛋白(a)水平仍与HOMA-IR显著负相关(β=-0.00996,P=0.0267)。结论脂蛋白(a)水平可能是2型糖尿病患者胰岛素抵抗的独立预测因子,较高的脂蛋白(a)水平与改善的胰岛素敏感性相关。Objective To investigate the correlation between lipoprotein(a)levels and insulin resistance in patients with type 2 diabetes,and to explore its potential role in metabolic regulation of diabetes.Methods A single-center cross-sectional study was conducted involving 106 patients with type 2 diabetes.The participants were divided into two groups based on lipoprotein(a)levels:a high-level group(lipoprotein(a)>1oo mg/dL)and a low-level group(lipopro-tein(a)<1oo mg/dL).General information and laboratory indices,including lipoprotein(a),fasting plasma glucose(FPG),fasting insulin(FINS),and Homeostasis Model Assessment of Insulin Resistance(HOMA-IR),were collected.The correlation between lipoprotein(a)and HOMA-IR was evaluated using Spearman correlation analysis.A multivari-ate linear regression model was applied to verify the independent relationship between lipoprotein(a)levels and HO-MA-IR after adjusting for confounding factors.Results The HOMA-IR of the high-level lipoprotein(a)group was sig-nificantly lower than that of the low-level group(P=0.036).Correlation analysis indicated a negative correlation be-tween lipoprotein(a)levels and HOMA-IR(r=-0.26,P=0.0066).Regression analysis showed that,after adjusting for confounding factors such as age,gender,and Body Mass Index(BMI),lipoprotein(a)levels remained significantly negatively correlated with HOMA-IR(β=-0.00996,P=0.0267).Conclusion Lipoprotein(a)levels may serve as an independent predictor of insulin resistance in patients with type 2 diabetes,with higher lipoprotein(a)levels associated with improved insulin sensitivity.Further research is required to confirm causality and investigate the underlying mech-anisms,providing new perspectives for individualized management of diabetes.
