重庆市医疗机构静脉用药集中调配中心运行现状调查与分析

摘  要:目的 推动静脉用药集中调配中心(PIVAS)的标准化、规范化及同质化建设,提高其运行及管理质量。方法 根据《静脉用药集中调配质量管理规范》《静脉用药调配中心建设与管理指南(试行)》和重庆市的PIVAS运行情况设计调查问卷,问卷内容包括医疗机构等级、PIVAS建设规模及PIVAS的人员结构、配置概况、硬件设备、信息化建设、质量控制模式等,并通过问卷星进行线上问卷调查,采用Excel软件对回收问卷进行统计与分析。结果 参与调研的医疗机构共56家,其中已建成PIVAS的医疗机构共23家(占41.07%,三级医疗机构20家,二级医疗机构3家),其建成时间主要集中在2011年至2020年;PIVAS的建设使用面积与调配量基本适配;三级医疗机构PIVAS配置的药品种类更多;三级医疗机构主要收取了细胞毒类药物及肠外营养液两类药物的配置费,二级医疗机构尚未收取配置费;PIVAS的生物安全柜和水平层流台配置完善,但环境监测频次不尽相同;智能静脉用药配置设备配备率、信息化管理系统覆盖率及全院医疗系统数据连接率均较低;PIVAS医嘱审核工作开展较好,并同步开展合理用药相关服务工作;PIVAS岗位工作人员主要由药学专业技术人员、护理专业技术人员、工勤人员组成,具有多样性;各级医疗机构的PIVAS均建立了质量控制体系,并对工作人员开展了不同程度的培训与考核,但考核结果未与绩效挂钩。结论 重庆市医疗机构PIVAS的建设虽取得了一定成效,但医疗机构配置费收取率较低,环境监测频次、监测项目不足,智能化、信息化覆盖率不高,考核与绩效挂钩不够。应加强信息化和智能化建设,增加药学专业技术人员占比,完善绩效考核机制,以推动重庆市PIVAS的标准化、规范化及同质化建设,提高PIVAS运行及管理质量。Objective To promote the standardization,normalization and homogenization of the pharmacy intravenous admixture service(PIVAS),and to improve its operation and management quality.Methods A questionnaire was designed according to the Specification for Quality Management of Pharmacy Intravenous Admixture Service,Guidelines for the Construction and Management of Pharmacy Intravenous Admixture Service(Trial)and the actual operation of the PIVAS in Chongqing.The questionnaire included the level of medical institutions,the construction scale of the PIVAS,the personnel structure of the PIVAS,configuration,hardware equipment,information construction,and quality control mode.The online survey was conducted through the Questionnaire Star,and the returned questionnaires were statistically analyzed by the Excel software.Results A total of 56 medical institutions participated in the survey,of which 23 had constructed PIVAS(accounting for 41.07%,twenty tertiary medical institutions,and three secondary medical institutions),mainly constructed from 2011 to 2020.The area of PIVAS construction was basically compatible with the dispensing volume.Tertiary medical institutions had a greater variety of drugs in the PIVAS.Tertiary medical institutions mainly charged for the preparation of cytotoxic drugs and parenteral nutrition solutions,while secondary medical institutions had not yet implemented charges.The configuration of biological safety cabinets and horizontal laminar flow workstations in the PIVAS was well-equipped,but the frequency of environmental monitoring varied.The rate of intelligent intravenous drug preparation equipment,coverage of information management systems,and data connection rate with the hospital-wide medical system were all relatively low.The medical order review work in the PIVAS was carried out well,and rational drug use-related work was also conducted simultaneously.The composition of personnel in PIVAS positions was diverse,mainly composed of pharmaceutical professionals,nursing professionals,and general
