摘  要:目的探讨MRI血管壁成像在评估症状性大脑中动脉粥样硬化患者血管壁厚度和斑块分布评估中的应用价值。方法选取2022年4月至2024年4月医院收治的100例症状性大脑中动脉粥样硬化患者作为研究对象,依据弥散加权成像(DWI)检查结果分为急性脑梗死组(52例)与短暂性脑缺血发作组(48例)。两组均行MRI血管壁成像检查。比较两组标准化血管壁指数、平均血管壁厚度、最大血管壁厚度、斑块分布情况、斑块面积、血管面积(VAreference)、参考血管腔面积(LAreference)、血管最狭窄处面积(VAMLN)、血管最狭窄处管腔面积(LAMLN)及管壁面积(WAMLN)。结果两组标准化血管壁指数比较,差异无统计学意义(P>0.05)。急性脑梗死组的平均血管壁厚度、最大血管壁厚度均高于短暂性脑缺血发作组,差异有统计学意义(P<0.05)。两组斑块分布情况比较,差异无统计学意义(P>0.05)。急性脑梗死组斑块面积、WAMLN高于短暂性脑缺血发作组,LAMLN低于短暂性脑缺血发作组,差异有统计学意义(P<0.05);两组VAreference、LAreference、VAMLN比较,差异无统计学意义(P>0.05)。结论MRI血管壁成像可准确识别症状性大脑中动脉粥样硬化患者的血管壁厚度和斑块分布情况,有助于监控疾病进展,为临床医师提供精准的治疗决策参考。Objective To explore the application value of MRI vascular wall imaging in evaluating vascular wall thickness and plaque distribution.Methods With the selection of 100 patients with symptomatic middle cerebral artery atherosclerosis admitted to the hospital from April 2022 to April 2024 as the research objects,according to the results of diffusion weighted imaging(DWI),they were divided into acute cerebral infarction group(52 cases)and transient ischemic attack group(48 cases).With both groups undergoing MRI vascular wall imaging examination,the standardized vessel wall index,average vessel wall thickness,maximum vessel wall thickness,plaque distribution,plaque area,vessel area(VAreference),reference vessel lumen area(LAreference),narrowest vessel area(VAMLN),narrowest vessel lumen area(LAMLN)and wall area(WAMLN)were compared between the two groups.Results There was no statistically significant difference in the standardized vascular wall index between the two groups(P>0.05).The average and maximum vascular wall thickness of the acute cerebral infarction group were higher than those of the transient ischemic attack group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the distribution of plaques between the two groups(P>0.05).The plaque area and WAMLN in the acute cerebral infarction group were higher than those in the transient ischemic attack group,and the LAMLN was lower than that in the transient ischemic attack group,with statistical significance(P<0.05);The comparison of VAreference,LAreference,and VAMLN between the two groups showed no statistically significant differences(P>0.05).Conclusion Vascular wall thickness and plaque distribution in patients with symptomatic middle cerebral artery atherosclerosis could be accurately identified by MRI vascular wall imaging,which helped to monitor disease progression and provide accurate reference for clinicians to make treatment decisions.
