摘  要:目的分析2019-2023年广东省医疗机构介入放射学工作人员眼晶状体剂量监测情况。方法本研究采用方便抽样方法, 用热释光剂量测量法对2019-2023年广东省34家三级医疗机构介入放射学工作人员的眼晶状体剂量及胸部剂量进行监测, 对监测数据进行汇总分析, 并对两者之间关系进行探索。结果共监测介入放射学工作人员1 033人次, 监测眼晶状体剂量范围在[<最低探测水平(MDL)~26.12 mSv]之间, M(P_(25),P_(75))为0.03(<MDL, 0.20)mSv, 剂量<MDL、MDL~、1.25 mSv~、5 mSv~、10 mSv~、20 mSv~分别为428、543、48、5、6、3人次, 分别占总监测人次的41.43%、52.57%、4.65%、0.48%、0.58%、0.29%;各年眼晶状体剂量范围分别为(<MDL~0.42)mSv、(<MDL~4.95)mSv、(<MDL~26.12)mSv、(<MDL~10.66)mSv、(<MDL~5.41)mSv;各年剂量M(P_(25),P_(75))分别为<MDL(<MDL, <MDL)mSv、<MDL(<MDL, 0.04)mSv、0.06(<MDL, 0.16)mSv、0.04(<MDL, 0.24)mSv、0.08(<MDL, 0.27)mSv;神经介入, 心血管介入、消化介入、肿瘤介入、外周血管介入、综合介入工作人员眼晶状体剂量范围分别为(<MDL~3.85)mSv、(<MDL~13.77)mSv、(<MDL~1.38)mSv、(<MDL~1.24)mSv、(<MDL~4.54)mSv、(<MDL~26.12)mSv, 剂量M(P_(25),P_(75))分别为0.05(<MDL, 0.16)mSv、0.07(<MDL, 0.28)mSv、0.04(<MDL, 0.09)mSv、0.03(<MDL, 0.03)mSv、<MDL(<MDL, 0.07)mSv、0.05(<MDL, 0.32)mSv;不同类别介入放射工作人员眼晶状体剂量比较差异有统计学意义(H=49.85,P < 0.01), 剂量较高的是综合介入、心血管介入放射工作人员, 较低者为肿瘤介入放射工作人员;经Spearman相关性检验, 介入放射学工作人员眼晶状体及胸部剂量相关呈显著性(R=0.37, P<0.001);经线性回归函数拟合得出关系式:眼晶状体剂量=1.044 ×胸部剂量。结论广东省医疗机构介入放射学工作人员眼晶状体剂量监测结果大致良好, 不同工种、不同介入类别工作人员接受剂量不同, 心血管介入和综合介入工作人员是改善放射防护工作的重Objective To study and analyze radiation doses to the eye of the lens dose in interventional radiology staff of Guandong medical institutions from 2019 to 2023.Methods Employing a convenience sampling method,the lens dose and chest dose was monitored with TLD dosimeters in 34 level A tertiary medical institutions.The monitoring data were analyzed on the basis of a summary,and the relationship between the two was explored.Results A total of 1033 interventional radiology staff were monitored.The ocular lens doses were in the range of(<MDL-26.12 mSv),M(P_(25),P_(75))was 0.03(<MDL,0.20)mSv.The doses<MDL,MDL-,1.25 mSv-,5 mSv-,10 mSv-,20 mSv-were monitored for 428,543,48,5,6,and 3 times,respectively,accounting for 41.43%,52.57%,4.65%,0.48%,0.58%and 0.29%of total monitoring times,respectively.The ocular lens doses each year was,seperately,in the range of(<MDL-0.42)mSv,(<MDL-4.95)mSv,(<MDL-26.12)mSv,(<MDL-10.66)mSv,(<MDL-5.41)mSv.M(P_(25),P_(75))were<MDL(<MDL,<MDL)mSv,<MDL(<MDL,0.04)mSv,0.06(<MDL,0.16)mSv,0.04(<MDL,0.24)mSv,0.08(<MDL,0.27)mSv,respectively.The eye lens doses to the staff involved in neural intervention,cardiovascular intervention,digestive intervention,tumor intervention,peripheral vascular intervention and comprehensive intervention was(<MDL-3.85)mSv,(<MDL-13.77)mSv,(<MDL-1.38)mSv,(<MDL-1.24)mSv,(<MDL-4.54)mSv,(<MDL-26.12)mSv.Dose M(P_(25),P_(75))were 0.05(<MDL,0.16)mSv,0.07(<MDL,0.28)mSv,0.04(<MDL,0.09)mSv,0.03(<MDL,0.03)mSv,<MDL(<MDL,0.07)mSv,and 0.05(<MDL,0.32)mSv.The difference of ocular lens dose in staff among different interventional procedures was statistically significant(H=49.85,P<0.01).The higher doses were observed in staff in cardiovascular interventional radiology and comprehensive intervention.The lower doses were received by staff involved in tumor interventional procedures.The correlation between eye lens and chest dose were significant,as showm by spearman correlation test(R=0.37,P<0.001).The corelation fitted by linear regression function follows the formula:eye lens dose=1.044×chest
