佛山市顺德区医院输血科运营现状调查与分析

摘  要:目的 通过对佛山市顺德区医疗机构的输血科(血库)建设情况及输血相关业务的现状进行调查、分析,为卫生健康行政部门和医疗机构对临床输血管理决策与输血科规范化建设提供依据。方法 通过发放调查表的方式,对顺德区内21家临床用血医院进行调查,内容包括医疗机构基本情况、2022年度医疗机构业务开展情况、输血科(血库)业务开展情况及人员设备配置情况等。结果 不同类别医院中无独立输血科有专职人员、无独立输血科无专职人员的设置情况对比,差异有统计学意义(P<0.05);各类别医院中独立输血科设置情况对比,差异无统计学意义(P>0.05)。在输血人次及血液制剂的用量上,三级医院远远高于二级医院;不同类别医院血小板保存震荡仪、血栓弹力图检测仪、全自动血型仪、输血信息管理系统配置情况对比,差异有统计学意义(P<0.05);融浆机与血细胞分离机不同医院类别对比,差异无统计学意义(P>0.05)。仅2家开展疑难交叉配血相关检测技术及血小板抗体筛查鉴定技术;7家医院开展自体输血技术,开展率仅为33.3%,且全区总的自体输血率仅为11.0%,仅有3家医院开展输血治疗项目,其中2家为输血科开展。结论 顺德区输血科建设总体落后于医院发展,输血科专职人员配备不足,血液相关检测技术未能全面开展,检测器械配备不全。Objective This study investigated and analyzed the construction of blood transfusion departments(blood banks)and the current status of blood transfusion related operations in Shunde District,Foshan City,to provide a basis for health administration departments and medical institutions to make decisions regarding clinical blood transfusion management and standardization of blood transfusion department construction.Methods A total of 21 hospitals with clinical blood use in Shunde District were investigated through the distribution of questionnaires regarding the medical institutions’basic status,the business development of medical institutions in 2022,the business development of blood transfusion departments(blood banks),and personnel and equipment allocation.Results Statistically significant differences were observed in settings with no independent blood transfusion department and no independent blood transfusion department(P<0.05).No significant differences were observed in the setting of independent blood transfusion departments at different hospitals(P>0.05).Tertiary hospitals had much higher numbers of blood transfusions and amounts of blood preparations than secondary hospitals.Statistical differences were observed in the configurations of platelet preservation oscillators,thromboelastogram detectors,automatic blood group meters,and blood transfusion information management systems across hospital types(P<0.05).No statistical difference was observed between plasma melting machines and blood cell separators among hospital types(P>0.05).Only two hospitals used difficult cross-matching blood related detection technology and platelet antibody screening and identification technology.Seven hospitals used autologous transfusion technology:the implementation rate was only 33.3%,and the total rate of autologous transfusion in the region was only 11.0%.Only three hospitals conducted transfusion treatment projects,two of which were performed by the blood transfusion department.Conclusions The construction of blood trans
