紧急起始腹膜透析在伴睡眠障碍老年糖尿病肾脏病终末期患者的应用

摘　　要：目的:现今伴有睡眠障碍的老年糖尿病肾脏病终末期患者紧急起始肾脏替代治疗仍以血液透析优先,本研究的目的为探讨腹膜透析作为替代选择的可行性。方法:选取2021年8月至2023年8月间,厦门医学院附属第二医院肾内科紧急起始透析的老年糖尿病肾脏病终末期患者86例作为研究对象,按照随机数字表法随机分为腹膜透析组和血液透析组,每组43例。对2组间的透析充分性、匹兹堡睡眠质量评分、国际不宁腿评定量表及相关指标进行2个月随访比较。结果:对2组患者进行2个月随访,2组匹兹堡睡眠质量评分、国际不宁腿评定量表及相关实验室指标均较前改善,差异均无统计学意义(均P>0.05)。结论:紧急起始腹膜透析疗效与血液透析相当,可作为伴有睡眠障碍的老年糖尿病肾脏病终末期患者替代选择。Objective:To investigate the feasibility of peritoneal dialysis(PD)as an alternative to hemodialysis(HD)for urgent-start renal replacement therapy in elderly end-stage diabetic kidney disease patients with sleep disorders,where HD is traditionally prioritized.Methods:A total of 86 elderly end-stage diabetic kidney disease patients requiring urgent-start dialysis,admitted to the Department of Nephrology,Second Affiliated Hospital of Xiamen Medical College,from August 2021 to August 2023,were enrolled.The patients were randomly assigned to the PD group and the HD group,with 43 cases in each group.The adequacy of dialysis,Pittsburgh Sleep Quality Index(PSQI),International Restless Legs Scale(IRLS),and other related indicators were compared between the two groups over a two-month follow-up period.Results:After two months of follow-up,both groups showed improvements in PSQI,IRLS scores,and related laboratory indicators.There were no statistically significant differences between the two groups(P>0.05).Conclusion:Urgent-start peritoneal dialysis offers comparable efficacy to hemodialysis and can be considered a viable alternative for elderly end-stage diabetic kidney disease patients with sleep disorders.
