中医药治疗慢性主观性耳鸣伴焦虑状态研究进展

摘　　要：慢性主观性耳鸣伴发焦虑状态给患者工作和生活带来了巨大的困扰,但由于其发病机制尚未完全阐明,西医在治疗该疾病方面总体效果欠佳。中医治疗该疾病以整体观、辨证论治的思想为指导,采用中药方剂、针灸推拿等诸多内外合治的方式,在治疗该疾病方面具有明显优势。本文综合文献观点,认为慢性主观性耳鸣伴焦虑状态的发生主要与心、肝、脾、肾等脏腑密切相关其基本病机为虚实夹杂,实证以肝火上炎、肝郁气滞为主,虚证以心脾两虚、心肾不交、肾精亏虚多见。从中医药治疗、中西医结合治疗以及外治法3个方面的研究进展进行综述,以期为该类患者的临床治疗和药物研究提供参考,进一步提高其临床疗效。参考文献41篇。Chronic subjective tinnitus with anxiety has become a significant burden on patients,affecting both their work and daily life.As the exact pathogenesis has not been fully clarified,the effectiveness of western medicine in treating this condition has been limited.Guided by the thoughts of holism and syndrome differentiation and treatment,traditional Chinese medicine(TCM)employs Chinese medicinals,TCM formulas,acupuncture,moxibustion,tuina,and various treatment methods internally and externally to treat this disease,which have demonstrated notable advantages in managing this disorder.In this paper,viewpoints from the literature was summarized,and it was held that the occurrence chronic subjective tinnitus with anxiety state is closely related to heart,liver,spleen,kidney and other zang-fu organs,and the fundamental disease mechanism is mixed deficiency and excess.The excessive syndromes usually involve liver fire up stirring and liver qi stagnation,while the deficient syndromes involve deficiency of heart and spleen,heart and kidney failing to communicate,and deficiency of kidney essence.The literature was reviewed from Chinese medicinals treatment,integrated treatment by TCM and western medicine,and external treatment,totally 3 aspects,so as to provide a reference for the clinical treat and medicinal research of this disease,and to improve the clinical efficacy.A total of 41 research papers are cited in the references.
