达格列净联合沙库巴曲缬沙坦治疗老年急性心肌梗死患者介入治疗后并发心力衰竭的临床效果

摘　　要：目的 探讨达格列净联合沙库巴曲缬沙坦治疗老年急性心肌梗死(AMI)患者介入治疗后并发心力衰竭(HF)的临床效果。方法 选取2021年1月—2023年12月青岛市市北区人民医院收治的老年AMI介入治疗后并发HF患者70例,采用信封法将其分为观察组和对照组,各35例。对照组采用沙库巴曲缬沙坦钠片治疗,观察组在对照组治疗基础上加用达格列净片治疗。比较两组治疗6个月后临床疗效,治疗前和治疗6个月后超声心动图指标[左心室射血分数(LVEF)、心排血量(CO)和室间隔厚度(IVST)]、血清N末端B型利钠肽前体(NT-pro BNP)、肿瘤坏死因子-α(TNF-α)水平,以及治疗期间不良反应。结果 治疗6个月后,观察组临床总有效率高于对照组;两组LVEF和CO高于治疗前,IVST低于治疗前,且观察组LVEF和CO高于对照组,IVST低于对照组;两组血清NT-pro BNP和TNF-α水平均低于治疗前,且观察组低于对照组(P<0.05)。治疗期间,两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 达格列净联合沙库巴曲缬沙坦治疗老年AMI患者介入治疗后并发HF的临床效果显著,能改善患者心功能,调节血清NT-pro BNP和TNF-α水平,且具有一定安全性。Objective To investigate the clinical effect of Dapagliflozin combined with Sacubitril Valsartan in the treatment of heart failure(HF)after interventional therapy in elderly patients with acute myocardial infarction(AMI).Methods A total of 70 elderly patients with AMI complicated with HF after interventional therapy admitted to Shibei District People's Hospital of Qingdao from January 2021 to December 2023 were selected and divided into observation group and control group by envelope method,with 35 cases in each group.The control group was treated with Sacubitril Valsartan Sodium Tablets,and the observation group was treated with Dapagliflozin Tablets on the basis of the control group.The clinical efficacy after 6 months of treatment,echocardiography indexes[left ventricular ejection fraction(LVEF),cardiac output(CO)and interventricular septum thickness(IVST)],serum N-terminal pro-B-type natriuretic peptide(NT-proBNP)and tumor necrosis factor-α(TNF-α)levels before and after 6 months of treatment,and adverse reactions during treatment were compared between the two groups.Results After 6 months of treatment,the total clinical effective rate of the observation group was higher than that of the control group;LVEF and CO in the two groups were higher than those before treatment,and IVST was lower than that before treatment,and LVEF and CO in the observation group were higher than those in the control group,and IVST was lower than that in the control group;the levels of serum NT-proBNP and TNF-αin the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusions The clinical effect of combined treatment of Dapagliflozin and Sacubitril Valsartan in the treatment of HF in elderly patients with AMI after interventional therapy is significant.It can improve the cardiac function of patients,regulate the levels of serum NT-proBN
