摘　　要：目的 探讨针尖斜面测量法在维持性血液透析(MHD)患者动静脉内瘘穿刺中的应用效果。方法 选取2022年11月—2023年11月在黄河三门峡医院进行MHD治疗的210例自体动静脉内瘘患者为研究对象,根据治疗区域不同将其分为对照组(透析一区)和观察组(透析二区)各105例。对照组采用常规绳梯式穿刺,观察组采用针尖斜面测量法绳梯式穿刺。比较两组患者动静脉内瘘穿刺并发症发生情况、护士规范绳梯式穿刺执行率及患者穿刺满意度。结果 两组患者动脉瘤发生率比较,差异无统计学意义(P>0.05);观察组动静脉内瘘狭窄和穿刺点渗血发生率均低于对照组,护士规范绳梯式穿刺执行率和患者穿刺总满意度高于对照组,差异有统计学意义(P<0.05)。结论 针尖斜面测量法绳梯式穿刺能够降低MHD患者动静脉内瘘穿刺并发症发生率,提高护士规范绳梯式穿刺执行率和患者穿刺满意度。Objective To investigate the effect of needle tip slope measurement in maintenance hemodialysis(MHD)patients arteriovenous fistula puncture.Methods A total of 210 patients with autologous arteriovenous fistula who received MHD treatment in Yellow River Sanmenxia Hospital from November 2022 to November 2023 were selected as the study objects,and were divided into control group(dialysis zone 1)and observation group(dialysis zone 2)according to different treatment areas,with 105 cases in each group.The control group was treated with conventional rope ladder puncture,and the observation group was treated with needle tip slope measurement rope ladder puncture.The incidence of arteriovenous fistula puncture complications,nurses'standardized rope ladder puncture execution rate and patients'puncture satisfaction were compared between the two groups.Results There was no significant difference in the incidence of aneurysm between the two groups(P>0.05).The incidence of arteriovenous fistula stenosis and puncture point bleeding in the observation group were lower than those in the control group,and the performance rate of standardized rope ladder puncture by nurses and the total satisfaction of patients with puncture were higher than those in the control group,with statistical significance(P<0.05).Conclusions The needle tip slope measurement rope ladder puncture can reduce the incidence of arteriovenous fistula puncture complications in MHD patients,and improve the implementation rate of nurses'standardized rope ladder puncture and patient puncture satisfaction.
