腹腔镜手术治疗宫外孕的临床效果及对导尿管留置时间的影响

摘　　要：目的评估腹腔镜手术应用在宫外孕(EP)患者中的临床效果及对导尿管留置时间的影响。方法纳入2020年1月-2023年12月期间我院收治的60例EP患者,依据治疗方法分组,对照组30例行开腹手术,观察组30例行腹腔镜手术,评价组间临床指标、应激反应[皮质醇(COR)、血糖(GLU)、醛固酮(ALD)]、甲状腺激素[游离T4(FT4)、游离T3(FT3)]、炎症因子[白介素-6(IL-6)、白介素-4(IL-4)、干扰素-γ(IFN-γ)]、并发症。结果①与对照组比较,观察组各项临床指标均更佳,并发症更少,P<0.05;②术后,两组COR、GLU、ALD、IL-6、IL-4、IFN-γ均升高,FT4、FT3均降低,但是观察组COR、GLU、ALD、IL-6、IL-4、IFN-γ均低于对照组,FT4、FT3均高于对照组,P<0.05。结论对EP患者实施腹腔镜手术,可以减少导尿管留置时间等临床指标,缓解应激反应、炎症反应,减轻对甲状腺激素的影响,减少并发症。Objective To evaluate the clinical efficacy of laparoscopic surgery in patients with ectopic pregnancy(EP)and its impact on the duration of catheterization.Methods A total of 60 patients with EP admitted to our hospital from January 2020 to December 2023 were and divided into groups according to treatment methods.The control group(n=30)underwent open surgery,while the observation group(n=30)laparoscopic surgery.Clinical indicators,stress response[cortisol(COR),glucose(GLU),aldosterone(ALD)],thyroid hormones[free T4(FT4),free T3(FT3)],inflammatory factors[interleukin-6(IL-6),interleukin-4(IL4),interferon-γ(IFN-γ)],and complications were compared between the two groups.Results①Compared with the control group,the observation group had better clinical indicators and fewer complications(P<0.05).②After surgery,COR,GLU,ALD,IL-6,IL-4,and IFN-γ increased in both groups,while FT4 and FT3 decreased.However,COR,GLU,ALD,IL-6,IL-4,and IFN-γ in the observation group were lower than those in the control group(P<0.05),the FT4 and FT3 were higher than those in the control group(P<0.05).Conclusion Laparoscopic surgery for EP patients can improve clinical indicators such as catheter retention time,alleviate stress and inflammatory reactions,reduce the impact on thyroid hormones,and reduce complications.
