补肾通络方对肾虚血瘀型膝骨性关节炎患者全膝关节置换术后康复的研究

摘　　要：目的探究补肾通络方对肾虚血瘀型膝骨性关节炎(KOA)患者全膝关节置换术(TKA)后的康复效果。方法共纳入本院收治的100例KOA患者,时间为2022年1月-2024年1月,以随机数表法分为两组,即对照组(n=50)、观察组(n=50),两组均接受TKA术式治疗,对照组术后予以常规治疗,观察组在此基础上予以补肾通络方治疗,疗程为4周,比较两组疗效;于治疗前、治疗2周、4周时,比较两组疼痛度[视觉模拟评分法(VAS)]、骨代谢指标[骨钙素(BGP)、骨保护素(OPG)、软骨寡聚基质蛋白(COMF)]、膝关节功能[西安大略和麦克马斯特大学骨关节炎指数(WOMAC)]。结果与对照组(84.00%)比较,观察组(96.00%)总有效率更高(P<0.05);治疗后,两组VAS评分、WOMAC评分均下降,且观察组低于对照组(P<0.05);治疗后,两组BGP、OPG水平提高,COMF水平下降,且观察组变化更明显(P<0.05)。结论补肾通络方治疗肾虚血瘀型KOA患者TKA后的康复效果显著,能减轻膝关节疼痛度,改善骨代谢指标,促进膝关节功能康复。Objective To investigate the rehabilitation effect of Bushen Tongluo Formula on patients with kidney deficiency and blood stasis type knee osteoarthritis(KOA)after total knee arthroplasty(TKA).Methods A total of 100 patients with knee osteoarthritis(KOA)admitted to our hospital from January 2022 to January 2024 were randomly divided into two groups using a random number table method:a control group(n=50)and a observation group(n=50).Both groups received TKA surgery.The control group received routine treatment after surgery,while the observation group received kidney tonifying and meridian unblocking formula treatment on this basis for 4 weeks.The efficacy of the two groups was compared;compare the pain levels[Visual Analog Scale(VAS)],bone metabolism indicators[Osteocalcin(BGP),Osteoprotegerin(OPG),Cartilage Oligomeric Matrix Protein(COMF)],and knee joint function[Western Ontario and McMaster University Osteoarthritis Index(WOMAC)]between the two groups before treatment,2 weeks after treatment,and 4 weeks after treatment.Results Compared with the control group(84.00%),the observation group(96.00%)had a higher total effective rate(P<0.05);after treatment,both groups’VAS scores and WOMAC scores decreased,and the observation group was lower than the control group(P<0.05);after treatment,the levels of BGP and OPG increased in both groups,while the level of COMF decreased,and the changes in the observation group were more significant(P<0.05).Conclusion The treatment of kidney deficiency and blood stasis type KOA patients with kidney tonifying and meridian unblocking formula has a rehabilitation effect after TKA,reducing knee joint pain,improving bone metabolism indicators,and promoting knee joint functional recovery.
