摘　　要：目的 探讨3D镜像打印辅助个性化钩钢板治疗NeerⅡB型成人锁骨远端骨折的临床效果。方法回顾性分析2018年4月至2023年6月收治的23例NeerⅡB型成人锁骨远端骨折患者,患者均采用3D镜像打印辅助个性化钩钢板治疗。记录患者骨折术后愈合时间;统计手术切口感染、骨折延迟愈合/不愈合、肩锁关节创伤性关节炎、肩峰周围骨溶解、肩峰撞击等并发症发生情况;术前及术后1、6个月采用ConstantMurley肩关节功能评分量表进行临床疗效评价。结果 23例患者均获得随访;骨折愈合时间为术后8~14周,平均(10.53±1.76)周;无手术切口感染等并发症发生。术后6个月,患者的Constant-Murley肩关节功能评分量表各维度评分及总分高于术后1个月、术前(P<0.05);术后6个月,Constant-Murley肩关节功能评分量表评估结果:优22例,良1例。结论 3D镜像打印辅助个性化钩钢板治疗NeerⅡB型成人锁骨远端骨折具有促进肩关节功能恢复、并发症少等优点,是治疗不稳定成人锁骨远端骨折的良好方法。Objective To explore the clinical effect of 3D mirror printing assisted personalized hook steel plate in the treatment of NeerⅡB type adult distal clavicle fracture.Methods A total of 23 adult patients with NeerⅡB type distal clavicle fractures admitted from April 2018 to June 2023 was retrospective analyzed.All patients were treated with 3D mirror printing assisted personalized hook steel plate.The healing time of fracture after operation was recorded;the incidence of complications such as surgical incision infection,delayed union/nonunion of fracture,traumatic arthritis of acromioclavicular joint,osteolysis around acromion,acromion impact and so on were counted;the Constant-Murley shoulder function score scale was used to evaluate the clinical efficacy before operation and 1 and 6 months after operation.Results All 23 patients were followed up;the fracture healing time was 8-14 weeks,mean(10.53±1.76)weeks;no complications such as surgical incision infection occurred.At 6 months after operation,the scores of each dimension and total score of Constant-Murley shoulder function score scale were higher than those at 1 month after operation and before operation(P<0.05);at 6 months after operation,the Constant-Murley shoulder function score scale showed excellent in 22 cases and good in 1 case.Conclusion The 3D mirror printing assisted personalized hook steel plate in the treatment of NeerⅡB type adult distal clavicle fracture has the advantages of promoting the recovery of shoulder joint function and less complications.It is a good method for the treatment of unstable adult distal clavicle fracture.
