气管插管与视频喉罩插管在全麻腹腔镜下小儿腹股沟疝手术中的应用效果比较

摘　　要：目的 比较气管插管与视频喉罩插管在全麻腹腔镜下小儿腹股沟疝手术中的应用效果。方法 选取2022年3月至2023年3月焦作市人民医院收治的86例全麻腹腔镜下小儿腹股沟疝手术患儿为研究对象,按随机数字表法将其分为对照组(43例,气管插管)和观察组(43例,视频喉罩插管)。比较两组的应用效果。结果 观察组的插管时间短于对照组,插管成功率高于对照组(P<0.05)。插管时,观察组的收缩压、舒张压、心率低于对照组,血氧饱和度高于对照组(P<0.05);插管后2 min,观察组的心率低于对照组(P<0.05)。插管时、二氧化碳气腹时、手术结束时,观察组的气道峰压、气道密闭压均低于对照组(P<0.05)。两组的苏醒时间、手术时间、拔管时间比较,差异无统计学意义(P>0.05)。观察组的术后并发症总发生率低于对照组(P<0.05)。结论 相较于气管插管,视频喉罩插管用于全麻腹腔镜下小儿腹股沟疝手术中可提高插管成功率,缩短插管时间,维持术中血流动力学稳定,并降低术后并发症发生率。Objective To compare the application effects of endotracheal intubation and video laryngeal mask intubation in pediatric laparoscopic inguinal hernia surgery under general anesthesia.Methods A total of 86 children undergoing laparoscopic inguinal hernia surgery under general anesthesia in Jiaozuo People's Hospital from March 2022 to March 2023 were selected as the research objects.According to the random number table method,the children were divided into control group(43 cases,endotracheal intubation)and observation group(43 cases,video laryngeal mask intubation).The application effects of the two groups were compared.Results The intubation time of the observation group was shorter than that of the control group,and the success rate of intubation was higher than that of the control group(P<0.05).During intubation,the systolic blood pressure,diastolic blood pressure and heart rate of the observation group were lower than those of the control group,and the blood oxygen saturation was higher than that of the control group(P<0.05);at 2 min after intubation,the heart rate of the observation group was lower than that of the control group(P<0.05).During intubation,carbon dioxide pneumoperitoneum,and the end of the operation,the peak airway pressure and airway sealing pressure of the observation group were lower than those of the control group(P<0.05).There were no significant differences in recovery time,operation time and extubation time between the two groups(P>0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion Compared with endotracheal intubation,video laryngeal mask intubation in pediatric laparoscopic inguinal hernia surgery under general anesthesia can improve the success rate of intubation,shorten the intubation time,maintain intraoperative hemodynamic stability,and reduce the incidence of postoperative complications.
