运动平板试验联合动态心电图在女性冠心病诊断中的应用价值

摘　　要：目的 分析运动平板试验(TET)联合动态心电图(DCG)在女性冠心病诊断中的应用价值。方法 回顾性分析2020年1月至2022年12月我院收治的70例疑似冠心病的女性患者检查资料。所有患者入院后分别进行TET与DCG,以冠状动脉造影诊断结果为金标准(62例),比较TET与DCG单一应用与联合应用对冠心病的诊断效能。结果 TET诊断出真阳性46例,真阴性5例;DCG诊断出真阳性44例,真阴性5例;TET与DCG联合应用诊断出真阳性59例,真阴性2例。TET、DCG单一应用对冠心病的诊断准确度、灵敏度及特异度无显著差异(P>0.05);TET、DCG单一应用及联合应用对冠心病的诊断准确度分别为72.86%、70.00%、87.14%;TET、DCG单一应用及联合应用对冠心病的诊断灵敏度分别为74.19%、70.97%、95.16%;TET、DCG联合应用对冠心病的诊断准确度、灵敏度均显著高于单一应用,差异具有统计学意义(P<0.05)。结论 TET联合DCG诊断女性冠心病可取得较高诊断效能,为患者的早期治疗干预提供科学依据。Objective To analyze the application value of treadmill exercise test(TET)combined with dynamic electrocardiography(DCG)in the diagnosis of female coronary heart disease.Methods The examination data of 70 female patients with suspected coronary heart disease admitted in our hospital from January 2020 to December 2022 were retrospectively analyzed.TET and DCG were performed in all patients after admission.The results of coronary angiography were used as the gold standard(62 cases).The diagnostic efficacy of TET and DCG alone and in combination for coronary heart disease was compared.Results TET diagnosed 46 cases of true positive and 5 cases of true negative;DCG diagnosed 44 cases of true positive and 5 case of true negative;the combination of TET and DCG diagnosed 59 cases of true positive and 2 cases of true negative.There were no significant differences in the accuracy,sensitivity and specificity of single application of TET and DCG in the diagnosis of coronary heart disease(P>0.05);the diagnostic accuracy of TET,DCG single application and combined application for coronary heart disease was 72.86%,70.00%and 87.14%,respectively;the diagnostic sensitivity of TET,DCG single application and combined application for coronary heart disease was 74.19%,70.97%and 95.16%,respectively;the diagnostic accuracy and sensitivity of combined application of TET and DCG for coronary heart disease were significantly higher than those of single application,and the differences were statistically significant(P<0.05).Conclusion TET combined with DCG can achieve higher diagnostic efficacy in the diagnosis of female coronary heart disease,and provide scientific basis for early treatment and intervention of patients.
