摘　　要：目的 探讨人参补肺汤合保元汤辅助治疗慢性阻塞性肺疾病(COPD)(肺肾气阴两虚证)的效果及对miR-210、缺氧诱导因子-1α(HIF-1α)的影响。方法 选取2017年1月至2022年12月收治的300例COPD患者,将其随机分为对照组和观察组,各150例。对照组采用沙美特罗替卡松吸入气雾剂联合氨茶碱片治疗,观察组在对照组基础上加人参补肺汤合保元汤治疗。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的动脉血氧分压(PaO_(2))、miR-210高于对照组,动脉血二氧化碳分压(PaCO_(2))、HIF-1α水平低于对照组(P<0.05)。治疗后,观察组的第1秒用力呼气容积占预计值百分比(FEV_(1)%)、用力肺活量占预计值百分比(FVC%)、第1秒用力呼气容积/用力肺活量(FEV_(1)/FVC)高于对照组(P<0.05)。结论 人参补肺汤合保元汤辅助治疗COPD(肺肾气阴两虚证)的效果显著,不仅能够调节miR-210、HIF-1α水平,还能促进肺功能康复。Objective To investigate the effect of Renshen Bufei decoction and Baoyuan decoction in the adjuvant treatment of chronic obstructive pulmonary disease(COPD)(lung and kidney Qi and Yin deficiency syndrome)and its influences on miR-210 and hypoxia inducible factor-1α(HIF-1α).Methods A total of 300 patients with COPD admitted from January 2017 to December 2022 were randomly divided into control group and observation group,with 150 cases in each group.The control group was treated with salmeterol xinafoate and fluticasone propionate inhalation aerosol combined with aminophylline tablets,and the observation group was treated with Renshen Bufei decoction and Baoyuan decoction on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the partial pressure of oxygen in arterial blood(PaO_(2))and miR-210 in the observation group were higher than those in the control group,and the partial pressure of carbon dioxide(PaCO_(2))and HIF-1αlevel were lower than those in the control group(P<0.05).After treatment,the percentage of forced expiratory volume in one second and its predicted value(FEV_(1)%),forced vital capacity as a percentage of predicted value(FVC%)and forced expiratory volume in one second/forced vital capacity(FEV_(1)/FVC)in the observation group were higher than those in the control group(P<0.05).Conclusion Renshen Bufei decoction combined with Baoyuan decoction has a significant effect in the adjuvant treatment of COPD (lung and kidney Qi and Yin deficiency syndrome), it can not only regulate the miR-210 and HIF-1α level, but also promote the rehabilitation of lung function.
