温经汤加减联合隔姜灸治疗寒凝血瘀型痛经的临床效果

摘　　要：目的 分析温经汤加减联合隔姜灸治疗寒凝血瘀型痛经的临床效果。方法 选择2023年1月至5月我院收治的40例寒凝血瘀型痛经患者作为研究对象,以治疗方案差异将其分为对照组和研究组,各20例。对照组给予布洛芬缓释胶囊口服治疗,研究组接受温经汤加减联合隔姜灸治疗。比较两组的治疗效果。结果研究组的治疗总有效率高于对照组(P<0.05)。治疗前,两组的疼痛视觉模拟量表(VAS)评分、疼痛持续时间及中医证候积分比较,差异无统计学意义(P>0.05);治疗后,研究组的VAS评分、中医证候积分低于对照组,疼痛持续时间短于对照组(P<0.05)。治疗前,两组的凝血酶原时间(PT)、国际标准化比率(INR)、活化部分的凝血活酶时间(APTT)、搏动指数(PI)及阻力指数(RI)比较,差异无统计学意义(P>0.05);治疗后,研究组的PT短于对照组,INR、PI及RI低于对照组,APTT长于对照组(P<0.05)。结论 温经汤加减联合隔姜灸治疗寒凝血瘀型痛经患者的效果显著,有助于改善疼痛症状,调节血流动力学平衡,值得临床应用。Objective To analyze the clinical effect of modified Wenjing decoction combined with ginger moxibustion in the treatment of dysmenorrhea of cold coagulation and blood stasis type.Methods From January to May 2023,40 patients with dysmenorrhea of cold coagulation and blood stasis type admitted in our hospital were selected as the research objects.According to the difference of treatment plan,the patients were divided into control group and study group,with 20 cases in each group.The control group was treated with ibuprofen sustained-release capsules orally,and the study group was treated with modified Wenjing decoction combined with ginger moxibustion.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the study group was higher than that in the control group(P<0.05).Before treatment,there were no significant differences in Visual Analogue Scale(VAS)score,pain duration and TCM syndrome score between the two groups(P>0.05);after treatment,the VAS score and TCM syndrome score of the study group were lower than those of the control group,and the pain duration was shorter than that of the control group(P<0.05).Before treatment,there were no statistically significant differences in prothrombin time(PT),international normalized ratio(INR),activated partial thromboplastin time(APTT),pulsatility index(PI)and resistance index(RI)between the two groups(P>0.05);after treatment,the PT of the study group was shorter than that of the control group,the INR,PI and RI were lower than those of the control group,and the APTT was longer than that of the control group(P<0.05).Conclusion Modified Wenjing decoction combined with ginger moxibustion in the treatment of dysmenorrhea patients with cold coagulation and blood stasis type has significant effect,which is helpful to improve pain symptoms and regulate hemodynamic balance.It is worthy of clinical application.
