纽曼系统护理模式联合营养支持对食管癌患者围术期营养状况及并发症发生情况的影响

摘　　要：目的 探讨纽曼系统护理模式联合营养支持对食管癌患者围术期营养状况及并发症发生情况的影响。方法 选取2020年7月至2022年10月于我院诊治的126例食管癌患者为研究对象,以随机数字表法将其分成观察组、系统组,各63例。两组均拟行食管癌根治术及常规药物治疗,同时观察组接受常规营养支持干预,系统组在此基础上予以纽曼系统护理模式联合营养支持干预。比较两组的并发症发生情况、胃肠功能恢复情况、营养状况、疼痛程度、负性情绪及生存质量。结果 系统组的并发症总发生率为4.76%,低于观察组的17.46%(P<0.05)。系统组的肛门排气时间、首次排便时间及首次下床时间短于观察组(P<0.05)。干预后,系统组的血清白蛋白(ALB)、转铁蛋白(TRF)及血红蛋白(Hb)水平高于观察组(P<0.05)。干预后,系统组的视觉模拟评分法(VAS)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分低于观察组,欧洲癌症研究与治疗组织生存质量核心量表(EORTC QLQ-C30)评分高于观察组(P<0.05)。结论 纽曼系统护理模式联合营养支持应用于食管癌患者围术期中,不仅能缓解患者疼痛程度、负性情绪,提升营养状况,促进胃肠功能恢复,还可显著提升生存质量,降低并发症发生率。Objective To explore the effects of Newman system nursing mode combined with nutritional support on perioperative nutritional status and complications of patients with esophageal cancer.Methods A total of 126 patients with esophageal cancer diagnosed and treated in our hospital from July 2020 to October 2022 were selected as the research objects.The patients were divided into observation group and system group by random number table method,with 63 cases in each group.Both groups were treated with radical resection of esophageal cancer and routine drug therapy.At the same time,the observation group received routine nutritional support intervention,and the system group was given Newman system nursing mode combined with nutritional support intervention on this basis.The incidence of complications,recovery of gastrointestinal function,nutritional status,pain degree,negative emotion and quality of life were compared between the two groups.Results The total incidence of complications in the system group was 4.76%,which was lower than 17.46%in the observation group(P<0.05).The anal exhaust time,the first defecation time and the first time to get out of bed in the system group were shorter than those in the observation group(P<0.05).After intervention,the levels of serum albumin(ALB),transferrin(TRF)and hemoglobin(Hb)in the system group were higher than those in the observation group(P<0.05).After intervention,the scores of Visual Analogue Scale(VAS),Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)in the system group were lower than those in the observation group,and the score of European Organization for Research and Treatment of Cancer Quality of Life Core Questionnaire 30(EORTC QLQ-C30)was higher than that in the observation group(P<0.05).Conclusion The Newman system nursing mode combined with nutritional support applied in the perioperative period of patients with esophageal cancer can not only alleviate the pain degree and negative emotion of patients,improve the nutritional status,promote the recover
