高龄胆总管结石患者行腹腔镜胆总管切开取石术的安全性分析

摘　　要：目的探讨高龄胆总管结石患者行腹腔镜胆总管切开取石术的安全性。方法回顾性分析我院2015年1月至2023年6月收治的胆总管结石且行腹腔镜胆总管切开取石的患者,将患者分为高龄(≥70岁)组和非高龄(<70岁)组。结果高龄患者在术前:高血压、术前肺部疾病、心脏疾病、ASA评分和术后:肺部感染、总费用、住院总时间、术后住院时间上与非高龄组存在显著差异(P<0.05)。结论腹腔镜胆总管切开取石是一种安全有效的治疗高龄胆总管结石患者的方法。Objective To investigate the safety of laparoscopic common bile duct exploration in elderly patients with common bile duct stones(≥70 years old).Methods Patients with common bile duct stones who underwent laparoscopic common bile duct exploration in our hospital from January 2015 to June 2023 were retrospectively analyzed,and the patients were divided into elderly(≥70 years old)group and non-elderly(<70 years old)group.Results A total of 148 patients were included in the study,with a maximum age of 92 years,including 54 patients in the elderly group and 94 patients in the non-elderly age group.The results of univariate analysis showed that elderly patients had significant differences between non-elderly disease in preoperative:hypertension,preoperative lung disease,heart disease,ASA score and postoperative:pulmonary infection,total cost,total hospital stay,and postoperative hospital stay(P<0.05).Elderly patients with common bile duct stone postoperative concurrent pulmonary infection risk factor analysis found:diabetes,hypertension,preoperative lung disease,heart disease,cerebrovascular disease these separate factors are not elderly laparoscopic common bile duct exploration postoperative concurrent pulmonary infection risk factors,ASA score>Ⅱis a risk factor for pulmonary infection in elderly patients with common bile duct stones after Laparoscopic common bile duct exploration.Conclusion Laparoscopic common bile duct exploration can be considered a safe and effective treatment for elderly patients with common bile duct stones.
